2855195 Primary Facility 2019-03-18 Routine/Complete Inspection No Violations Noted On March 18, 2019, Department representative Brad Kline conducted a routine site inspection at Capital Oil & Gas COP 678 65 well. No E&S issues were observed during the time of inspection. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover. There is a fiberglass brine tank on site that is disconnected from the well. The Department recommends that if the brine tank is no longer needed that it be removed from the site. The chart recorder was dated 3/7/19.

The Department screened the well head with an Altair 5x Multi-gas Meter. The screening returned a reading of 15 percent by volume methane, an over range reading for percent LEL, and a 23 ppm carbon monoxide.

No violations noted. This inspection is being referred to the Oil and Gas inspector for the area, due to gas detection around wellhead.
