No one was present during my inspection. During my inspection I walked the proposed injection well pad location. No earth disturbance has occurred to date. I did not observe any flagging or markings for this proposed injection well pad. This injection well site does not meet the 5 acre threshold therefore this project does not require an ESCGP. Please keep in mind best management practices (BMP) should be applied prior to earth disturbance to help reduce the amount of erosion and sedimentation. The Department would appreciate notification prior to any bulk earth disturbance for this project. Please contact me at 814-342-8130.
2619889 Primary Facility 2017-07-27 Routine/Complete Inspection No Violations Noted Site inspection of the Povlik O&G injection well (033-27257) on July 27, 2017. No one was present during my inspection. During my inspection I walked the proposed injection well pad location. No earth disturbance has occurred to date. I've reached out to the operator via phone on July 28 requesting a timeframe of when this project will start. I'm currently waiting a return call.

2679443 Primary Facility 2018-01-08 Routine/Complete Inspection No Violations Noted Routine site inspection of Sammy-Mar, LLC Povlik 1 injection well on January 8, 2018. No one was onsite during my inspection. I viewed the proposed injection well pad location during my inspection. To date, no earth disturbance has occurred. The ground was snow covered. I did not observe any E&S controls in place. This injection well site falls under the 5 acre threshold, therefore does not require an ESCGP but does require an E&S plan which they have.

The Department would appreciate a phone call prior to the operator conducting any bulk earth disturbance. I may be reached at 814-342-8130.

2699196 Primary Facility 2018-02-27 Routine/Complete Inspection No Violations Noted Routine site inspection at Sammy-Mar, LLC Povlik 1 injection well (033-27257) on February 27, 2018. No earth disturbance activities have occurred to date. Water samples from nearby streams were collected at 4 different locations surrounding the proposed injection well site. Samples were sent to the Departments Lab in Harrisburg for analysis under SAC 0046.

On February 28, 2018 I spoke with Chantelle of High Pointe Oil and Gas which is an affiliate of Sammy-Mar LLC. According to Chantelle, there is no tentative date set for drilling this injection well.

2721245 Primary Facility 2018-04-18 Routine/Complete Inspection No Violations Noted Department staff Sue Herzing and Sasha Minium conducted a routine site inspection at Sammy-Mar, LLC Povlik 1 injection well (033-27257) on April 18, 2018. No earth disturbance activities have occurred to date. Please note the well permit is approaching its expiration date and will expire on 5/30/18. If the operator plans on drilling after the well permit expires, they will need to apply for a new well permit.

No violations noted.

2730711 Primary Facility 2018-05-16 Routine/Complete Inspection No Violations Noted Department staff Sue Herzing and Ulyana Trynovich conducted a routine site inspection at Povlik 1 injection well (033-27257) with owner/operator Dan Sinclair of Sammy-Mar LLC on May 16, 2018.

The drilling permit is due to expire on 5/30/18. The operator plans on spudding the well and setting a 24" conductor 20' in the ground prior to the expiration date of the well permit. Mr. Sinclair has hired Butch's Rat Hole & Anchor Service out of Washington, PA to spud and set the conductor. According to Mr. Sinclair, the 20' drilling rig and 40' trailer will be stationed on secondary containment with a berm. Compost sock will be put in place directly down gradient of the injection well. Once the well is spud, the conductor with be set in place and sakrete will be placed around the outside of the casing. Once completed, the area will be backfilled, seeded, and mulched. According to Mr. Sinclair, they tentatively plan on drilling the rest of the injection well in July-August 2018.

2736573 Primary Facility 2018-05-29 Routine/Complete Inspection No Violations Noted The weather is 85 Deg. F and sunny at the time of inspection. Routine inspection of Sammy-Mar LLC., C.O.P. Povlic Pad, well # 1 on May 29, 2018.

20 ft. deep conductor has been drilled on May 24th, 2018. The area around the well has been preliminarily restored, planted grass seeds and silt sock has been placed. Residential water well is located at approximately 230 feet from the well. 0% combustible gas was detected in the area immediately surrounding the well. Discussed water sampling and testing with the camp owner, Mr. John Jeffrey Povlic, and company rep. present during inspection. No obvious concerns or violations are noted at time of inspection. 2739632 Primary Facility 2018-06-08 Routine/Complete Inspection No Violations Noted Department staff Sue Herzing conducted a routine site inspection at Sammy-Mar, LLC Povlik 1 injection well (033-27257) on June 8, 2018. The 24" conductor has been installed and is covered with a metal plate. The metal plate has the well permit number listed on it. 12" compost sock remains in place down gradient of the set conductor (see photo). All disturbed areas have been seeded and mulched. Germination is starting in the recently seeded area. No E&S issues observed.

2778470 Primary Facility 2018-09-21 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing spoke with owner/operator Dan Sinclair via phone on the morning of September 21, 2018. Mr. Sinclair was inquiring as to whether or not he needed to drill and/or alter the well by the end of September 2018. Mr. Sinclair had the conductor drilled and set on May 24, 2018. The well permit was due to expire on May 30, 2018. Mr. Sinclair has a drilling rig lined up to start drilling well 033-27257 on Tuesday, September 25, 2018. I have provided Mr. Sinclair with the Departments Oil & Gas Inspector Supervisor Mike Panettieri contact information.

I conducted a routine site inspection at Sammy-Mar LLC injection well 033-27257 on the afternoon of September 21, 2018. A contractor (Multi-Production Systems Inc.) was onsite moving soils. The contractor has installed 12" compost sock down gradient of the proposed injection well site. I recommended to Mr. Sinclair during the previous phone conservation, that some sort of liner with a berm be put in place where the rig is going to be

2780223 Primary Facility 2018-09-26 Follow-up Inspection No Violations Noted The weather is 68 Deg. F and raining at the time of inspection. Follow up inspection of Sammy-Mar LLC., C.O.P. Povlic Pad, well # 1 on September 26, 2018. Arrived to location due to drilling activity notification from ¿ Mr. Dan Sinclair, Sammy-Mar LLC. representative. Previously, 20 ft. deep conductor has been drilled on May 24th, 2018.

Did not observe any drilling activity on site. 0% combustible gas was detected in the area immediately surrounding the well. Two dozer machines are parked on site. The area around the well has been preliminarily restored, planted grass seeds and silt sock has been placed. DEP Representative - Ulyana Trynovich, Oil and Gas Inspector.

2781648 Primary Facility 2018-09-28 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at Sammy-Mar LLC Povlik injection well 033-27257 on September 28, 2018. The well pad and cuttings pit has been constructed. The liner for the cutting pit has not been installed but the liner is stationed onsite. The contractor has partially seeded and mulched the cut and fill slopes. 12" compost sock remains in place.

At 1445 on September 28, 2018 I called the owner/operator Dan Sinclair. Mr. Sinclair will not be bringing a rig onsite as previously discussed due to the permit expiring on May 30, 2018 and the due diligence period ending on September 30, 2018. Mr. Sinclair will be submitting all necessary paperwork to the Department in the upcoming weeks to obtain a new drilling permit for this site. Mr. Sinclair also indicated that he is all signed up through the Departments Greenport system now.

During the first week of October 2018, contractor will be reinstalling the property owners gas line and then will finish grading, mulching,

2786409 Primary Facility 2018-10-09 Routine/Complete Inspection No Violations Noted Routine site inspection on 10/09/18. All E&S controls remain in place per the E&S plan and appear to be functioning as design. All disturbed areas have been seeded and mulched with some seed germination observed. Several inches of water were observed in the open drill cuttings pit. As mentioned in my previous inspection report, the Department recommends installing some sort of barrier around this open pit, in efforts to eliminate any safety hazards.

2856949 Primary Facility 2019-03-20 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection on March 20, 2019 at Sammy-Mar injection well 033-27257. All E&S controls remain in place per the E&S plan and appear to be functioning as design. All disturbed areas have been seeded and mulched with some seed germination observed. I recommend that the operator monitor seed growth and re-seed if needed. Several inches of water were observed in the open drill cuttings pit. As mentioned in my previous inspection reports, the Department recommends installing some sort of barrier around this open pit, in efforts to eliminate any safety hazards.
