2686457 Primary Facility 2018-01-23 Routine/Complete Inspection No Violations Noted Routine site inspection at EQT Production Company's Turkey Well Pad on January 23, 2018. No one was onsite durning the time of my inspection. There are a total of 5 producing Marcellus wells drilled on this well pad (033-27049, 033-27051, 033-27053, 033-27055, and 033-27057).

The pad was originally permitted for 11 wells but only 5 of the 11 have been drilled to date.

All 5 wells are tagged with the API/well # and each well is plumbed to a gas production unit which include metering instrumentation, heater, and fluids separator. Well cellars are covered in metal grates, cellars are approximately half full, no oil sheen or bubbling was observed. There are (5) 400 bbl produced fluids tanks located in secondary containment. Containment appears intact. Approximately 2 inches of water was observed in this containment with a field screen reading of 1989 uS/cm.

The well pad has been reduced in size. The original design included (2) lined reserve/cutting pits along the entire SW edge of the Turkey Pad. These pit

2723067 Primary Facility 2018-04-24 Routine/Complete Inspection No Violations Noted I met Steve Shiner and Lance Beloney of Energy Water Solutions when I arrived. The VOX unit was not running; Steve informed me that the volume of water being produced by the wells had allowed them to run the unit for 5 days of 1200 gallon batches, but that there wasn't enough produced water in the production tanks to run a batch that day. So far, the unit has been able to evaporate approximately 20% of each batch before solids began to drop out. The stack test for the RFD is currently scheduled for May 15th. I noted two tanks, both empty, that were not in secondary containment; a 35 gallon propylene glycol tank next to one of the GPUs, and a double tank of methanol near the dehy unit. Although these tanks were empty, they had been in use recently; I recommend that these kinds of tanks be placed in secondary containment in the future. At the production tank containment, I noted that all of the 90 degree elbows on the feed lines to the tank were heavily corroded; one in particular (see photo) appeared to
2851664 Primary Facility 2019-03-11 Routine/Complete Inspection No Violations Noted The weather is 45 Deg. F, windy and sunny at time of inspection. Routine inspection of EQT Production Co., Turkey Pad, Well # 591242 on March 11, 2019. There are 5 active wells on this pad.

This well is open and producing at the time of inspection. Both annular vents (production by intermediate and intermediate by surface) are open and vented to the atmosphere. Unable to verify annular vents for gas using gas meter due to annular vent pipes are being raised up to unreachable height without a ladder. 0% combustible gas was detected in the area immediately surrounding the well. Production tubing pressure is 474 PSI, backside casing pressure 159 PSI. Production processing equipment on location includes 5 production water tanks, 5 gas processing units, 1 sand trap, measurement equipment ¿ orifice meter stations. No violations are noted at time of inspection, but DEP recommends labeling annular vents. DEP Representative - Ulyana Trynovich, Oil and Gas Inspector, Melissa Sue Herzing, Water Quality Specialist.
