2749379 Primary Facility 2018-07-06 Incident- Response to Accident or Event Violation(s) Noted Department representative Sue Herzing conducted an incident related inspection on July 6, 2018 at Devonian Resources conventional well 033-26494 (Walls 3 Well). A small spill of 15 gallons of brine water was reported to the Department on July 2, 2018 by Joe Thompson of Devonian Resources.

During the inspection I observed a small hole located near the top portion of the poly brine and orange staining down the side of the tank. Wet soil was present during my inspection. I used a soil probe to test for conductivity throughout the wet areas and dry areas. I estimated that the impacted area near the brine tank is 12 ft. wide and bottle necks down to 1.5 ft. wide as it travels down gradient for approximately 60 ft. The wet saturated areas maxed out the soil probe at 4.0 mS/cm. Some standing water was observed and maxed out the water screening conductivity meter at 20.0 mS/cm.

In addition, stressed vegetation consisting of evergreen trees and grasses was observed within the brine release pathway.

2768535 Primary Facility 2018-08-28 Follow-up Inspection Outstanding Violations - Viols Req'd Department representatives Sue Herzing and Sasha Minium conducted a follow up inspection on August 28, 2018 at Devonian Resources conventional well 033-26494 (Walls 3) with Joe Thompson of Devonian Resources. A small spill of 15 gallons of brine water was reported to the Department on July 2, 2018.

To date, the operator has only removed some of the impacted soils. Excavated impact soils remain onsite, wrapped in plastic. The bullet hole within the brine tank has been repaired. Impacted soils still remain present exceeding soil conductivity readings of 1,000 uS/cm. During the site inspection, discussions were had on remediating the site. The operator will be addressing remediation by further excavating impacted soils delineated by the Department during the site inspection. Once excavation is completed, please notify the Department. Impacted soils shall be properly disposed.

The previous cited violations on July 6, 2018 remain outstanding. No new violations were cited during this inspection. The outstanding

2822756 Primary Facility 2019-01-04 Follow-up Inspection No Violations Noted Department representatives Sue Herzing conducted a follow up inspection on January 4, 2019 at Devonian Resources Inc. conventional well 033-26494 (Walls 3). A small spill of 15 gallons of brine fluids was reported to the Department on July 2, 2018 by the owner/operator Joe Thompson.

Excavated impacted soils remain onsite and wrapped in plastic. The bullet hole within the brine tank has been repaired.

The Department recommends removing the impacted soils wrapped in plastic from the site and properly disposing of them. The Department is requesting a copy of the disposal receipts.

The previous cited violations on July 6, 2018 remain outstanding. No new violations were cited during this inspection. The outstanding violations will be closed upon receiving confirmation from the operator that the remedial activities have been completed and upon receiving disposal receipts.

2856846 Primary Facility 2019-03-21 Follow-up Inspection Outstanding Violations - Viols Req'd Department representatives Sue Herzing conducted a follow up inspection on March 21, 2019 at Devonian Resources Inc. conventional well 033-26494 (Walls 3). While onsite I meant will landowner Eric Perks. Mr. Perks stated that this well feeds to their nearby hunting camp located several hundred yards uphill from this location.

A small spill of 15 gallons of brine fluids was reported to the Department on July 2, 2018 by the owner/operator Joe Thompson. Excavated impacted soils remain onsite and wrapped in plastic. Serval holes and/or tears were observed in the plastic. A field soil probe reading was taken where a tear in the plastic occurred; reading > 4.00 mS/cm.

Since this small spilled occurred over 8+ months ago the Department has been recommending removing the impacted soils wrapped in plastic from the site and properly disposing of them.
