2740407 Primary Facility 2018-06-08 Routine/Complete Inspection No Violations Noted A routine inspection of this conventional, shallow gas well was conducted June 8, 2018. The purpose of this inspection is to evaluate the well and location for a potential upcoming plugging project. The well is approximately 50- 75' west of Burkett road. There is no access road leading directly to the well. The area leading up to the well is generally clear with a few small trees surrounding the SE corner of the well. The well has a master valve with a tee on top, valve on the side of the tee and a production line that immediately turns underground (see attached photo). No other onsite equipment was observed.
2802722 Primary Facility 2018-11-09 Routine/Complete Inspection No Violations Noted A routine pre-plugging inspection of a conventional well. The well is scheduled to be plugged under state contract OOGM 18-3. No one was onsite and no work has been done onsite at this time. No obvious issues were observed at surface.
2820445 Primary Facility 2018-12-28 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (plugging) inspection of this shallow, conventional gas well was conducted December 28, 2018. This well is the first of three wells being plugged under state contract OOGM 18-3. Plants & Goodwin are on location rigging up a spudder rig for final plug and abandonment. I met with the plugging contractor to discuss operations and the tentative procedure. The well is classified as a "coal" well and will be plugged as such. Well pressure prior to downhole operations is reportedly 30 PSI. The crew expects to be finished rigging up, on bottom, and spotting cement next Thursday, tentatively. No obvious issues were observed or reported at this time.
2822255 Primary Facility 2019-01-03 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (plugging) inspection of this shallow, conventional gas well was conducted January 3, 2019. This well is the first of three wells being plugged under state contract OOGM 18-3. Plants & Goodwin are rigged up for final plug and abandonment. A pump truck is on location to mix, pump, and spot cement. I met with the rig operator to discuss operations on location since my last inspection.They ran in the hole and tagged at 1388'; the string became stuck, however, the crew managed to free the string. It was found there was an obstruction in the open-hole section of the well and the decision was made to spot cement plug from 1156', above the obstruction, to avoid complications pulling out of the hole after spotting cement and still allowing for zonal isolation well below and at the casing shoe. I met with the pump truck operator to discuss the cementing procedure and fluid volumes. 92 bbls was calculated to load hole/establish circulation, followed by 92 sks of cement mixed and pumped as 19.4 bbls of cement
2822493 Primary Facility 2019-01-04 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (plugging) inspection of this shallow, conventional gas well was conducted January 4, 2019. This well is the first of three wells being plugged under state contract OOGM 18-3. Plants & Goodwin are rigged up for final plug and abandonment.

The well was open at the time of inspection, following yesterday's cementing operation. I spoke with the rig operator; they tagged solid cement at 997' earlier. They are now preparing to run in the hole with a bailer to empty the casing of liquid as described in 25 PA Code Chapter 78.92 (3) relating to plugging a well in a coal area where surface casing is cemented, and intend to install a vent atop the plugged well thereafter as also described in this regulation.

At the time of inspection, there was no audible or visible venting, bubbling, or discharge observed at the well. No gas was detected with an Altair 5x gas meter at the wellhead flange above master valve. There were no obvious issues observed at this time.

2824433 Primary Facility 2019-01-09 Administrative/File Review No Violations Noted January 9, 2019: Certificate of Well Plugging for the PA Parcel #3 (1-A) (033-20242) has been received, reviewed, signed, and sent to file. This is the first of three wells being plugged under state contract OOGM 18-3.
2827235 Primary Facility 2019-01-11 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, post-plugging, inspection of this plugged conventional gas well was conducted January 11, 2019. All personnel and equipment are off location following final plug and abandonment. At the time of inspection, a 2" vent pipe has been installed on the well atop existing wellhead flange. There was no audible or visible venting observed at the vent. No gas was detected with an Altair 5x gas meter at the mouth of the vent. No obvious issues were observed at this time. This is the first of three wells being plugged under state contract OOGM 18-3.
