2488959 Primary Facility 2016-05-25 Routine/Complete Inspection No Violations Noted On site at 12:08. Access is via Carlin Mining Rd, off Clarence Rd. Weather conditions at the time of the inspection were sunny and warm. No company representatives were present during the inspection. One well had been drilled on this site, Carlin 1. Excessive erosion or wash-out was observed during the inspection (see photos). The well and casing were situated within a large hole. The bottom of this hole was not visible from the surface. An Altair 5X multigas meter (calibrated on 05/23/16) was used to monitor for the presence of combustible gas in the hole. No combustible gas was detected at the time of the inspection. Access to the location is by foot. There is an old path adjacent to the field where the well is located, but the road is blocked on both sides by saplings and trees. Although the requirement for site or well identification for this well is currently being evaluated, no violations were noted at the time of the inspection. Left location @ 12:37.
