2823011 Primary Facility 2018-12-20 Routine/Complete Inspection No Violations Noted The weather is 40 Deg. F and cloudy at the time of inspection. Routine inspection of Capital Oil and Gas Inc., C &K Coal Co. Pad, Well # on December 20, 2018.

There is 1 producing well on this pad. Annulus vent (8-5/8" x 4-1/2") is open and vented. 0% combustible gas was detected at the annulus, but observed leaking fluid. 100% LEL and 8% combustible gas has been detected at the disconnected flow line. The valve at this line is shut in. Production line pressure is 15 PSI. Production equipment includes a drip and inline heater, production water tank; measurement equipment - orifice meter station. Observed wetland area forming around the well, will send a follow up referral to DEP WQS. DEP recommends applying new coating system and/or paint on wellhead and production equipment on location in order to prevent further corrosion. No violations are noted at time of inspection, but will perform a follow inspection regarding gas leak and annulus vent discharging fluid. Capital Oil and Gas Inc. Representative ¿ Dou

2825807 Primary Facility 2019-01-11 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at Capital Oil & Gas Inc. conventional well 027-20424 on January 11, 2019. This case was referred to me by a Departments Oil & Gas Inspector regarding the nearby wetland.

During my inspection I observed the well head, brine tank, and axillary piping for the production equipment. The well head may be located in a wetland with standing water present, but at the time of inspection, it was not possible to determine whether a wetland exists or not due to frozen conditions. The Department recommends that the operator get in touch with Oil & Gas surface permitting should work or additional earthmoving be necessary at this site, as additional permitting may be needed in regards to this potential wetland.
