2583964 Primary Facility 2017-04-12 Routine/Complete Inspection No Violations Noted On April 12, 2017 I conducted I routine (pre-plugging) inspection on Samson’s 2H, 3H, 12H, 13H, and C5 on the Pardee and Curtin Lumber Co. lease in Cameron county with Jerry Worsham of Samson to check pressures on the wells before plugging to evaluate how each well will need to be addressed in a proposed plugging procedure. The 12H and 13H had no major issues observed during the inspection. The 2H and 3H were bubbling in the cellar (violations pending post-plugging evaluation). No violations were noted on the C5, which was bubbling last inspection turned out to be an open vent that was underwater for backside of production casing. I put a gauge on the vent; no pressure, less than a trace of flow.
2616551 Primary Facility 2017-07-18 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of the Pardee C-12H conducted July 18, 2017 at 11:30 am. Rig is on location for plugging ops. CIBP and cement plug set in production casing. Production casing not recoverable. They perf’d at 1600’ and pressured up to 2500 PSI, could not get injection. Tried again at 1590’, 1580’, could not inject. Talked to operator about plan going forward. They’ll set a balanced plug across the perfs, then perforate at 1140’ and try to get injection rate. If successful they’ll pump a straight well-lock resin squeeze at this interval.
2624251 Primary Facility 2017-08-09 Plugging(Includes Plugged/Mined Through) Pending Inspection of the Pardee 2H conducted August 9, 2017 at 11 am. Rig is on location for plugging ops. The rig could not get injection behind production casing, even after last attempt to break circulation around packer set between sets of perfs. Packer is currently stuck in production casing. After they fish the packer out they will proceed to mill window across shoe and set balanced plug out to formation. I checked the 12H and 13H, the two wells the plugging rig skidded from. The 12H was venting gas, inflated balloon and exceeded 5” manometer water differential reading. The 13H was open to balloon, no inflation or flow observed.

2638966 Primary Facility 2017-09-19 Routine/Complete Inspection No Violations Noted Routine site inspection of Samson Exploration Pardee Curtain Pad E on September 19, 2017. No one was onsite during my inspection. The four wells on this pad have been recently plugged. The wells remain in an observation phase. The wells are not deemed plugged officially until the Department issues a plugging certificate. Well 023-21034 has some bubbling occurring within the well cellar (see picture). Joe Gaboff with the Department is over seeing the plugging operation and I will be referring the issue to Mr. Gaboff for further investigation.

Miscellaneous equipment from the plugging operation remains onsite. Some of the BMPs for site restoration have been installed.

2639976 Primary Facility 2017-09-25 Plugging(Includes Plugged/Mined Through) Pending Cellar full of water. Backside of production casing open and venting below water, bubbling. Rig is on other pad, operator intends to return to this well to remediate and complete P&A.
2663581 Primary Facility 2017-11-22 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine inspection of 4 plugged wells. Rig off location following remediation of the #12 to address annular venting. At the time of inspection this well had a 2" candy cane. No venting was observed, and there was no gas was detected with an altair 5x gas meter at the vent. The other 3 wells all had vent pipes as well, and no gas was detected. No obvious concerns were observed.

2670218 Primary Facility 2017-12-08 Administrative/File Review No Violations Noted December 13, 2017: Certificate of Well Plugging for the Pardee and Curtin Lumber Co C-12H (023-20134) received, reviewed, signed, and sent to file.
2696017 Primary Facility 2018-02-20 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine inspection of (4) plugged wells on pad. All four wells had 2" candy cane vent pipes erected as well monuments. No obvious integrity issues were observed.

All four wells need to be marked with permit number in accordance with 25 PA Code Chapter 78.97

2721410 Primary Facility 2018-04-23 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of four plugged unconventional wells on the Pardee and Curtin Lumber C12 pad was conducted April 23, 2018. At the time of inspection, the 12H, 13H, 2H and 3H all had 2" candy cane vent pipes with marker sign erected as a plugged well monument. No visible or audible flow was observed at the vents. All casings are below grade and the pad is reclaimed. No obvious major issues were onserved at this time.
2745010 Primary Facility 2018-05-29 Routine/Complete Inspection No Violations Noted Routine site inspection at Pardee & Curtin Pad E on May 22, 2018 with Michale Race and Joe Kunst of ARM Group and Steve Trujilo of Samson. Samson requested an inspection to evaluate the vegetation status and whether the site meets the requirements for a notice of termination. A few bare spots were observed with little vegetation, but overall the site meets 70% vegetative cover. All run-off from the site is being directed towards the wetland as designed by the restoration plans. I recommend, pulling the skin off the compost sock and spreading the mulch prior to submitting the notice of termination for ESX10-023-0001. The Department recommends submitting as built drawings with the notice of termination.

2759701 Primary Facility 2018-08-01 Routine/Complete Inspection No Violations Noted On behalf of Samson Exploration, ARM Group Inc. has submitted a notice of termination (NOT) for Pardee Curtin CO C-12 (Well Pad E) covered under ESX10-023-0001. The Department received this NOT on July 16, 2018.

On August 1, 2018 Department representative Sue Herzing conducted a restoration inspection at the above listed site with Steve Trujilo of Samson Exploration and Joe Kunst of ARM Group. The well pad has been restored back to approximate original contours. All run-off from the site is being directed towards the wetland. We walked throughout the restored well pad during the site inspection. Overall the site exceeds 70% uniform, perennial, vegetative cover. All temporary E&S controls have been removed.

Four Marcellus wells have been plugged (023-20134, 023-20135, 023-20185, and 023-20186). Each well is represented by a tagged well monument. Post plugging site restoration reports have been submitted for the above listed wells.
