This well pad is not covered under an ESCGP; it falls under the 5 acre threshold. There is one Marcellus well (023-20123) located on this well pad. This well has recently been plugged and is represented by a tagged well monument.

Samson will be restoring this site under an E&S plan. This pad was built on relatively flat ground and will be restored back to approximate original contour. During my inspection I observed 2 roll off containers and a frac container which looks like it was used to hold cementing materials. In addition, I observed some ruts throughout the well pad. No sediment was observed leaving the site.

E&S BMP controls will need to be installed prior to any earth disturbance work.

2663482 Primary Facility 2017-11-21 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine inspection of a plugged well. Rig off location following re-plug. Well has 2" candy cane vent, open and venting. Approximately 10 % gas was detected at the vent opening. No gas was detected outside the vent and no bubbling was observed in standing water around well. As I went on to inspect other wells on the lease I called the operator to arrange meeting to further evaluate this well. I met the operator approximately one hour later at the 7H. A 1-160 PSI pressure gauge was fitted to the well and monitored for several minutes. The well did not move the needle of the pressure gauge and the volume when removing the gauge was minimal, bled off in approximately one second or less.

This well has recently been plugged and is represented by a tagged well monument.

Samson has restored this site under an E&S plan. This pad was built on relatively flat ground and has been restored back to approximate original contour. Some miscellaneous materials remain onsite from the plugging operation. I recommend removing these materials. Some rutting within the restored area was observed. No sediment was observed leaving the site. No violations noted.

2687599 Primary Facility 2018-01-26 Routine/Complete Inspection No Violations Noted Routine site inspection of Samson Explorations Well Pad C (Pardee & Curtin Lumber Co C7H) on January 19, 2018. This well pad is not covered under an ESCGP; it falls under the 5 acre threshold.

There is one Marcellus well (023-20123) that was plugged during late fall 2017. This plugged well is represented by a labeled well monument (photo 1).

The site has been restored under an E&S plan. The pad was built on relatively flat ground and has been put back to approximate original contour (photo 2). The site has been seeded and mulched. Some large ruts were observed on the restored pad during my inspection. I recommend regrading the rut areas once weather permits and then reseeding and mulching.

A frac container stationed in secondary manmade containment remains onsite (photo 3). I recommend removing this frac container and containment materials weather permitting.

2695981 Primary Facility 2018-02-20 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine inspection of a recently plugged well. The well has a 2" candy cane monument with well tag. No obvious major issues were observed.
2721276 Primary Facility 2018-04-23 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this plugged, unconventional well was conducted April 23, 2018. At the time of inspection, all casing is below ground level. There is a 2" candy cane vent extending 6' above ground level. There was no audible or visual flow coming from the vent. There was standing water on the pad next to the location of the well. No bubbling was observed. There were no obvious major issues observed.

This plugged well is represented by a labeled well monument (photo 1).

The site has been restored under an E&S plan. The pad was built on relatively flat ground and has been put back to approximate original contour (photo 2). Several large ruts and bare spots were observed. The Department recommends dressing up any rutted areas and reseeding bare spots. In addition, the Department recommends submitting a post plugging restoration report in accordance with Chapter 78a65(f).

2759438 Primary Facility 2018-08-01 Site Restoration No Violations Noted A well site restoration inspection of Samson Explorations Well Pad C (Pardee & Curtin Lumber Co C7H) was conducted on August 1, 2018 with Steve Trujilo of Samson and Joe Kunst of ARM Group. A post plugging site restoration report has been submitted for plugged well 023-20123. This well was plugged during late Fall 2017. This plugged well is represented by a tagged monument.

The construction of the well pad was not under an ESCGP; it falls under the 5 acre threshold. The site has been restored under an E&S plan. The pad was built on relatively flat ground and has been put back to approximate original contour. Large ruts observed during my last inspection in June have been addressed. Those areas have been regraded, more soil added, seeded and mulched. Germination on these areas is occurring.

Per this inspection I approve the post plugging restoration report for 023-20123.
