1884716 Primary Facility 2010-05-11 Drilling/Alteration No Violations Noted According to well records, spud called on May 7, 2010 for May 10, 2010 spud date. Stephenson contracted Whipstock to drill conservation well to a depth of 9800 ft. Well sign and E&S plan present at the site. E&S controls were present on the down gradient side of site with filter fencing. Drilling pit aligned with 30 mil plastic. No freeboard issues. No signs of leakage of gas around wellhead. At the time of inspection, no E&S issues and no IW issues. Current drilling status was completed cementing of 11 3/4¿ pipe 250 sks with 3 % CASCL2. Pipe set at 421.45 ft. Conductor pipe set at 36 ft. Permit and drillers log were present at the site along with receipts of cement slips showing 16:30 hrs of completion of cement. Whipstock was not drilling at the time and was allowing the cement to set. No circulation or cement return was noted on cement slip. Stratigraphy log showed ss, sh, limestone.
1885812 Primary Facility 2010-05-19 Drilling/Alteration No Violations Noted According to well records, spud called on May 7, 2010 for May 10, 2010 spud date. Stephenson contracted Whipstock to drill conservation well to a depth of 9800 ft. Well sign and E&S plan present at the site. E&S controls were present on the down gradient side of site with filter fencing. Drilling pit aligned with 30 mil plastic. No freeboard issues. No signs of leakage of gas around wellhead. At the time of inspection, no E&S issues and no IW issues. Current drilling status was completed cementing of 8 5/8¿ pipe 485 sks with 3 % CASCL2. Pipe set at 1602 ft. Cement return of 24 bbl. Permit and drillers log were present at the site along with no receipts of cement slips. Whipstock was at 4655 ft. 7.875 ¿ hole was going to 9290 depth. Stratigraphy log showed ss, and sh. Water pond was added to site since last visit on May 11th located topographic down gradient of mud and drilling pits. Plastic was being held by excavator and needed support
1888013 Primary Facility 2010-05-20 Routine/Complete Inspection No Violations Noted At the time of inspection, rig was located onsite and well was being drilled. Site was properly signed with E&S plan at entrance. It is recommended that operator include a more detailed E&S plan map for future sites, major modifications also need to be approved by the dept. No accelerated E&S issues were noted at the time of inspection in relationship to well site or access road but, is recommend additional access road bmps be installed & additional rock be added due to the steep nature of the roadway; runoff is a potential concern Observed the proper installation functioning of filter fabric fence below grade of well site. It is recommended that the soil pile either be seeded or additional bmps are installed surrounding the pile to minimize E&S. Four below grade pits were located onsite freeboard levels ok, it is recommended that operator continue to monitor and maintain the required 2ft of lined freeboard and ensure that all pit liners are properly secured with backfill or stakes. Noted: water pit was temporarily secured with heavy equipment.
1888920 Primary Facility 2010-06-01 Complaint Inspection No Violations Noted At the time of inspection, roadways (intersection of Glendale Lake Rd and ST Augustine Rd) leading to/from well site were muddy. Spoke with Darryl of Woodstock while onsite, who was going to skim road with skid loader to remove mud, which was underway prior to dept leaving site. The dept recommended that a broom truck also be utilized to clean the roadways. A follow up inspection will be completed to ensure compliance. Once at the site, access road entrance was not properly rocked and was muddy. Rig was no longer onsite, well drilled not yet set for production. Site did not exhibit any accelerated erosion or sedimentation issues.
1888926 Primary Facility 2010-06-03 Follow-up Inspection No Violations Noted This was a follow up inspection to a complaint inspection performed on Tues June 1, 2010 for muddy roadways coming and out of the well site. Upon inspection, noted that Glendale Lake Road and St. Augustine Road had been successfully cleaned. Appears that all mud had been removed and roadways were swept. Operator complied with the department¿s requests in an efficient and courteous manor. No accelerated E&S issues were observed. It is recommended that the operator continue to maintain the access road, as well as all roadways leading to the well during all phases of construction in order to prevent such an event from occurring in the future.
1918394 Primary Facility 2010-10-07 Routine/Complete Inspection No Violations Noted At the time of this inspection, well drilled, not yet equipped for production. Site properly signed with ES plan available for review. All pits have been removed and site has been regarded and seeded and mulched. Growth is exhibited, but 70% uniform coverage has not yet been sustained. Filter fence installed and functioning properly. No accelerated erosion or sedimentation issues observed. Access road adequately rocked and pad size effectively reduced to minimize final area of disturbance. Until final restoration measures have been completed and >70%unfiorm growth is established operator should continue to monitor all bmp¿s.
1961967 Primary Facility 2011-03-22 Site Restoration No Violations Noted At the time of inspection, well drilled not yet in production. Site ID sign present. Well completed 05/23/10. Site has been re-graded, pits removed. Site appears adequately restored with min 70% permanent vegetative cover, although reseeding is recommended to ensure all areas exhibit a uniform coverage of growth; no accelerated erosion or sedimentation issues noted. Temporary bmp¿s are to be removed upon final restoration. Access road stable.
2018060 Primary Facility 2011-11-07 Routine/Complete Inspection No Violations Noted At the time of inspection, well drilled. Drilling completed 5/23/10. Site ID sign present, although well tag illegible--recommend operator replace. Based on disturbed earth, appears pipleine may have been recently installed. Areas have beeen reseeded mulched. Well site uniformly restored; therefore, operator can remove filfter fence installed down gradient from pad. Access road stable and adequately rocked. No accelerated erosion or sedimention issues noted. Orange security fence located around well head. No violations noted at the time of this inspection.
