Department attempted to contact Pennzoil by phone number listed in Department records; number disconnected. Department recommends Operator permanently identify well with permit number. Department also recommends Operator update records to show correct Ownership and contact information of the plugged well site. No E&S issues observed at site located in the woods. It appears that access road has been removed and restored. Department unable to locate certificate of well plugging or restoration report. A follow up inspection will be conducted at a later date.
