2757196 Primary Facility 2018-07-24 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, Gary Northime had a rig at the well and tubing has been pulled. Well had around 1500' of 4 7/8" inside of 6 1/4" surface casing. There was also 8" conductor pipe. Packer on the 4 7/8" is set at 1497'. Well has been cemented from TD (1697') up to 1460'. 4 7/8" pipe was perforated at 1395', 1303', 1210', 1190', 993', and 898'. Plans are to shoot well and pull 800'.

2 plastic totes were onsite on a flat bed trailer and another tote was being used for cement mixing. Several cement sacks were on another trailer.

2760336 Primary Facility 2018-08-03 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, well has been plugged. Well had been cemented up since last inspection to 840'. 5 cuts were made in the 4 7/8" pipe and it popped when pulled. 820' of casing was pulled out, but pipe parted in 2. Top of casing was then found to be at 375'. Remaining pipe was perforated and cemented up to 342'. Pea gravel was then added to surface. An above ground marker was set as a 2" vent pipe that had a plate, with the API number welded on it, attached. An excavator remained onsite. The pump jack and separator remained near the well.
