Cellar for this well is set, but not cemented yet. This should take place today. Hole for conductor has not been started yet so well has not been technically spud yet. There is a tank onsite where cuttings are being placed. Permit and Emergency Response signs are posted at the entrance to the pad. Along the access road there are sound walls that are 2 panels high. Entering the well pad, there are sound walls 4 panels high in the one corner closest to houses.
2640670 Primary Facility 2017-09-20 Routine/Partial Inspection No Violations Noted At the time of inspection, conductor for well has been set. 24" was set to 40'. This has been cemented into place. Well is now covered with a steel plate. Mouse hole was set beside the well.

Cementers were lined up for noon. Rig and equipment were on containment. Chemicals and fuel were within double containment. Plan is to run 13 3/8" casing to 564' with a float shoe and float collar, as well as 5 centralizers.
2652306 Primary Facility 2017-10-19 Routine/Partial Inspection No Violations Noted At the time of inspection, Appalachian Drillers 1 was over the 5H well. This well had surface casing cemented using 510 sxs (111 bbls) Class A,2% CaCl2, 2% Gypsum, 15.6 ppg, 1.22 yield. Displaced with 83 bbls and landed plug. Floats held and 13 bbls cement returned to surface.

Intermediate casing was cemented using 695 sxs (151 bbls) Class A, 2% Gypsum, 2% CaCl2, 15.6 ppg, 1.22 yield. Displaced with 152 bbls freshwater and landed plug. Floats held and 25 bbls cement returned to surface. A float shoe and float collar (top at 1964') were used along with 16centralizers.

2680923 Primary Facility 2018-01-09 Routine/Partial Inspection No Violations Noted At the time of inspection, Nomac rig 79 is onsite over the #3H well. Production casing for this well was cemented using a lead of 185 sxs 35:65 Poz Class H (36.3 bbls), 15.2 ppg, 1.1 yield and tail of 955 sxs Class H (266 bbls), 15.6 ppg, 1.57 yield. This was displaced with 270 bbls freshwater. Plug was bumped and spacer was returned to surface. Floats held. Top of cement is at 5929'. A float shoe and float collar were used with 182 centralizers.

There were also a few roll offs and frac tanks on the pad. Containment has been removed and was sitting in the dumpsters. Site was covered with snow, but I did not see any indication of any environmental concerns. Well or pit were not covered and everything looked good.

Stimulation operations underway at time of inspection.

Inspected pad liner, secondary containments, AST, well development pipeline, E/S controls, waste management, and storage tanks.

Operator reported that flow back operations were underway at time of inspection and there were about 100 stages left in the stimulation.

I observed flow back water transfer operations for a period of time while on site.

Heavy rain event while on site.

2700521 Primary Facility 2018-02-22 Drilling/Alteration No Violations Noted At the time of inspection, C & J was onsite stimulating stage 17 of well. Currently, the stimulation is a single well job. There are a total of 35 stages that will be completed. Operations are running 24/7.

Several people in the area have reported either an odor or noise complaint. The odor was reported on Monday. The company man onsite had just come back from days off so he was unsure of operations on Monday. So I contacted his relief. He could not think of any unordinary odors. They had just started stimulation on Monday and he completed 2 stages and the night man did 3. He stated there were no spills and could only suggest diesel fumes from the trucks. In terms of the noise, XTO is working with the township to remedy the issue. They may be extending sound walls soon. There are 3 wells needing to be completed. The other 2 wells have 47 stages for the 3H and 44 for the 5H. Complaint ID is 332468.

2700522 Primary Facility 2018-02-27 Drilling/Alteration No Violations Noted At the time of inspection, C & J was onsite stimulating stage 2 of the well. There are a total of 47 stages that will be completed. Operations are running 24/7.

There have been no changes to operations and there has not been any additions to the sound walls yet. Drove to township building and wind was blowing towards the building from the well pad. This made sound from pad more prevalent. I do not have an accurate, calibrated noise reading device to determine the decibel levels. Complaint ID is 332468

2742623 Primary Facility 2018-06-14 Routine/Partial Inspection No Violations Noted At the time of inspection, well is still being worked on to be brought into production. Well has a Christmas tree installed on it, but the flow line has yet to be hooked up. There were some workers on the pad doing this, but it didn't appear that it would be completed for a few more days. There has be no earth moving activities yet. Cellar has been filled in to surface with gravel. 4 tanks and 3 line heaters have been placed within lined, steel dikes. The pipeline work is continuing off in the distance from the pad.
2760326 Primary Facility 2018-08-02 Routine/Partial Inspection No Violations Noted At the time of inspection, well remains shut in until pipeline is completed to the pad. Company representative stated that this could take a few more weeks. All of the piping needed to produce on the pad has been completed flow lines, emergency shut off valves). More stone had been added and reworked. 4 tanks are set within a lined, steel dike and 3 line heaters within another dike. A gas buster has also been installed.

No activity was taking place during inspection. There were 2 frac tanks that remained on the pad.

Well drilled, completed, and in production. Well tag in place. Well protected by fencing and jersey barriers. Production equipment set (production tanks and GPUs) in secondary containment. Combustor installed. Gathering pipeline installed.

Site stabilized, PCSM BMPs installed and 100% vegetated.
