1941087 Primary Facility 2010-12-28 Complaint Inspection No Violations Noted Free atmospheric methane was sampled for complaint investigation using a combustable gas meter.
1941107 Primary Facility 2010-12-30 Complaint Inspection No Violations Noted
1941114 Primary Facility 2010-01-04 Complaint Inspection No Violations Noted Free gas samples taking while investigating complaint of stray gas.
1943858 Primary Facility 2011-01-06 Complaint Inspection No Violations Noted This inspection involved taking free atmospheric gas samples with a combustable gas meter calibrated for % methane. 80% methane in old water well, 0% methane in new water well (used) and Cypher Gas Well vent was @ 98% methane.
1944522 Primary Facility 2011-01-19 Complaint Inspection No Violations Noted Free atmospheric gas samples were taken as part of investigation of this complaint. At the Cypher Well 1.4% methane. 0.0% at the "new" water well. 60% at the old water well. At the time of the inspection the vent was clogged due to frozen condensation in the line. The vent line was cut off to prevent the collection of condensation.

1944564 Primary Facility 2011-01-21 Complaint Inspection No Violations Noted
1944600 Primary Facility 2011-01-20 Complaint Inspection No Violations Noted Inspection done as part of an on-going gas migration complaint. Free Atmospheric Gas measurement. 0% in New Well 0% in Old Water Well. End of plastic vent line- 0%. Gas sample was not taken at the Cypher well head vent. No plume was observed at well head vent.
1945180 Primary Facility 2011-01-24 Complaint Inspection No Violations Noted
1946218 Primary Facility 2011-01-25 Complaint Inspection No Violations Noted Stray Gas Investigation inspection: Well vent was clogged. Called well operator BLX to reconfigure vent in a safe yet more effective manner that prevents blockage due to freezing and/or snow blockage. Gas samples were taken at old water well, new water well and at this gas well. New water well 0%, Old water well 24%, Cypher well vent 0%.methane.
1946223 Primary Facility 2011-01-13 Complaint Inspection No Violations Noted Stray Gas complaint investigation. Down hole work being conducted on this well to set plug and to remove stuck packer, etc... Atmospheric methane levels as taken with the MSA Model 60. Old water well= 0%, New Water Well= 0%, Cypher gas well vent= 98% methane.
1946226 Primary Facility 2011-01-14 Complaint Inspection No Violations Noted Inspection done as part of a stray gas investigation. Service rig on site. Well plug was set in the Cypher Well in the 3" casing. Free atmospheric gas samples were taken at the following locations: Old water well = 0% and New Well 0%. No sample taken at the Cypher well due to the fact work was being performed down hole at the time of inspection.
1946608 Primary Facility 2011-01-27 Complaint Inspection No Violations Noted Complaint inspection conducted as a result of an on-going complaint investigation for stray gas. BLX personel were contacted and informed that the existing 1.5" plastic vent line that they installed was clogged again. BLX said that they would reconfigure the vent in a safe manner and in a way that will prevent freezing/clogging. Old water well vent was reconfigured vertically to prevent freezing. Free gas readings taken with the MSA model 60 are as follows Old Water Well= 0 %, New Water Well = 0%. New vent configuration appears to be working.
1946623 Primary Facility 2011-01-26 Complaint Inspection No Violations Noted Inspection was done as part of an on-going complaint investigation. BLX staff were contacted and informed that the vent line on the old Crytzer water well was plugged. The 1.5" line was most likely clogged by frozen condensation. Free Atmospheric gas readings are as follows. Old water well= 54%, New Water Well 0%, Old water well vent 0%.
1946629 Primary Facility 2011-01-31 Complaint Inspection No Violations Noted This inspection was done as part of a on-going stray gas investigation. New vertical vent in place on well head. Y was installed by operator to make sampling easier. Atmospheric Free Gas reading were: Old water well= 0%, New Water Well= 0% and Cypher Well vent 2 %.
