2810556 Primary Facility 2018-12-05 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of Repsol Oil & Gas's Monro (03-142) G Well Pad in Columbia Twp, Bradford County on 12/5/2018 at 0830 hours. The weather was 19 degrees and partly cloudy.

Tory Rogers, site safety supervisor, showed me around the site. They are currently drilling on the 6H with the Patterson Rig 805 and just recently started. The 40 gallon cement spill on containment that occurred on 12/3/2018 appeared to be cleaned up and remediated (see photo #1). All fluids observed were frozen, no issues were noted. No erosional issues were observed. All containment looked to be functioning properly. They are using double walled containment around there frac tanks which appeared to be working well. (photo #3). Tory was able to email me some photos taken on the day of the cement spill and a map to show the location (see photos #5, 6, 7, & 8).

No violations noted at this time. I departed the site at 0920 hours.

2816857 Primary Facility 2018-12-18 Drilling/Alteration No Violations Noted Arrived onsite to conduct a drilling inspection, located and entered the company mans quarters, the company man was not present, i did not conduct a full inspection due to company man was not present at time of inspection. I spoke to security guard at entrance of well site, he informed me that the company man had just left. I will return at a later date to conduct a full inspection.
2842189 Primary Facility 2019-02-20 Routine/Complete Inspection No Violations Noted I spoke with Shawn Callahan (REM Security & Safety) on site, and spoke with Rudy Yoder by phone. They explained the following: Repsol is rigging down Patterson 805 after drilling 8 Marcellus Shale wells from this pad. This morning ~0930 approx 1/2 quart of oil based drilling mud spilled on the pad. They removed the mud, and some pad material (photo 2) this morning.

I did not take electrical conductivity readings because i did not see any standing liquid on site (all frozen). Air temp 25F, snowing. JLP
