2605837 Primary Facility 2017-06-14 Drilling/Alteration No Violations Noted On June 14, 2017, the Department conducted a routine inspection at the Repsol Oil & Gas USA Inc., Hemlock Valley (05-265) well site in Pike Township, Bradford County. Andrew Hobbins, Water Quality Specialist, and Jon Sassi, EPS, represented the Department. Johnny Pazzaglia was on site for Repsol.

Repsol is spudding ten wells at the Hemlock Valley well site to a depth of 195 ft. and 175 ft. Mouseholes associated with each well were drilled to a depth of 66 feet. Only one well remains to be spudded. Mr. Pazzaglia stated drilling would be completed by Thursday, June 15, 2017. Cuttings were being placed in roll-off containers and being sent to Chemung Landfill in Elmira, NY. Well cellars were observed on site and will be installed when the wells are spud and conductor casings have been set. There were no puddles or stains observed on the well pad surface.

2627932 Primary Facility 2017-07-26 Routine/Complete Inspection No Violations Noted On July 26, 2017, the Department conducted a routine inspection at the Repsol Oil and Gas USA LLC, Hemlock Valley well site in Pike Township, Bradford County. Andrew Hobbins Water Quality Specialist, represented the Department. Tory Rogers, Site Safety, accompanied the Department on the inspection. Jack Burns was the company man on site for Repsol. Patterson-UTI Drilling Company LLC Rig 805 was on site.

According to Mr. Rogers, drilling of the 6H well has been completed and they are currently drilling the 4H well and they were nearing the kick-off point during the inspection. The operator is using Soli-Bond material to stabilize and solidify drill cuttings. Three waste containers were staged on rig mats over secondary containment during the inspection. Drill cuttings are being taken to the Chemung Landfill for disposal.

On 7/20/2017, the Department was notified of a small spill of hydraulic oil on to rig mats that are on top of secondary containment. The Department observed the area to be clean

2637448 Primary Facility 2017-08-29 Routine/Complete Inspection No Violations Noted On August 29, 2017, the Department conducted a routine inspection at the Repsol Oil and Gas USA LLC, Hemlock Valley well site in Pike Township, Bradford County. Andrew Hobbins Water Quality Specialist, represented the Department. Tory Rogers, Site Safety, accompanied the Department on the inspection. Patterson-UTI Drilling Company LLC Rig 805 was on site.

According to Mr. Rogers, drilling of the 3H well was being drilled during the inspection. A total of five wells have been drilled to completion. The operator is using Soli-Bond material to stabilize and solidify drill cuttings. Drill cuttings are being taken to the Chemung Landfill for disposal.

On 8/28/2017, the Department was notified of a release of approximately 7-10 gallons of anti-freeze from a truck that was delivering casing to the well pad. The truck apparently blew a radiator line and leaked the fluid onto the township road and onto the access road to the pad. 2653984 Primary Facility 2017-10-11 Routine/Complete Inspection No Violations Noted I arrived at the site at 12:00. The weather conditions were cloudy skies with light rain falling and temperatures in the 60's. Patterson-UTI Rig #805 is on the location. The most recent Drilling Resumption After Break of30 days or more notification submitted by Repsol to the Department was on 10/2/17. The notice indicated Repsol would resume drilling of the Hemlock Valley 8H well on 10/3/17 at 12:00 PM. I met with Tory Rogers, Site Safety Man of REM Security, Jack Burns, site company man, Petar Malenchia, Drilling Engineer both of Repsol, and TJ Fuller, Directional Driller of Slumberge. The current drilling depth is (12,884 feet). Repsol stated the following in reference to site activities. "We are currently in the process of drilling the horizontal section of the 8H well. The well is being drilled with synthetic oil based mud. The well will be drilled to total depth which is at (19,700) feet. This is the last well to be drilled on this pad for the time being. 2668141 Primary Facility 2017-11-28 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 11:01. The weather conditions are sunny and breezy with temperatures near 40 degrees. Halliburton is the Repsol subcontractor on-site in the process of conducting hydraulic fracturing (stimulation) operations. Repsol submitted a stimulation notification to the Department on 11/20/17. The notice indicated the date and approximate time action to occur was on 11/24/17 at 6 AM. I met with Sheridan Anderson, site company man, completions consultant of Repsol and Ralph Babcock, site safety man of REM security. Mr. Anderson stated the following regard the spills and site activities. Zipper frac is in progress. We are on stage (5) on the 10H well. Frac is 24/7. We will frac 1H-10H wells. Spill #1 is a (10) gallon spill of friction reducer mixed with freshwater and production fluid that occurred on 11/26/17 at 3:30 AM. I reported the spill to DEP NCRO at 8 AM on the 26th. Blender tub overflowed fluid to containment as the pump was left on. Spill #2 occurred on 11/27/17 at 6:30 AM. I reported
2675798 Primary Facility 2017-12-20 Routine/Complete Inspection No Violations Noted On December 20, 2017, the Department conducted an incident response inspection at the Repsol Oil and Gas USA Inc., Hemlock Valley Well Pad in Pike Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Sheridan Anderson, Company Man, was on site for Repsol. The Department was notified of two separate spills onto secondary containment by Sheridan Anderson.

The Department was given a tour of the site. The two spill areas were observed. The first spill involved production fluids that leaked onto secondary containment due to a faulty gasket on a boost pump. Approximately 8 gallons of production fluid was released. Repsol had a vac truck immediately remove the fluid from containment. The second spill involved oil leaking from a series of oil filters due to hoses rubbing and vibrating against the filters. The oil was removed from containment, however some residual oil remained. The Department recommends cleaning up the remaining oil on containment.

2679489 Primary Facility 2017-12-28 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 10:53 in response to a reported spill of (100) gallons of hydraulic fracturing fluid to secondary containment. The weather conditions were partly cloudy skies, windy and frigid with temperatures 5-10 degrees Fahrenheit and wind chill values below zero. I met with Sheridan Anderson, site company man, completions consultant of Repsol in the company man trailer. Halliburton is the Repsol subcontractor in the process of conducting hydraulic fracturing (stimulation) operations. Mr. Anderson informed me of the following information in reference to the spill and site activities. "Zipper frac is in progress now, on stage 22 of 43 for the 2H well. We are rigged up to the 2H, 4H and 6H wells. The 5, 8 and 10 wells are completed. The 1, 3, 5 and 7 are not. It is a (24) hour frac. The spill happened at 6:15 AM today. (100) gallons of frac fluid was spilled to containment out of the blender. The frac fluid in the blender is a mixture of freshwater from well development impoundments, the above gro
2687208 Primary Facility 2018-01-09 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 11:32 in response to reported spills of (5) gallons of biocide and (200) gallons of fluid from a tank containing from Repsol well development impoundments. The weather conditions were mostly cloudy skies, quite windy, with temperatures in the 30's. I met with Dell Gunn, site company man, completions foreman, consultant of Repsol, and Ralph Babcock, Site Safety Man of REM Security. Mr. Gunn and Mr. Babcock informed me of the following information pertaining to site activities and the spills. "Frac is in progress. Halliburton is on stage 36 of 38 on the 2H well. The first spill was (5) gallons of biocide. It occurred on 1/6/18 at 11 AM. The biocide spilled onto containment out of a chemical trailer. A hose popped out of a port in the trailer. Maybe the cam lock fitting was loose. A vac truck sucked up the fluid immediately and put it back into the trailer. Around 5AM on 1/7/18 water in the Freshwater DYNA tank froze at a certain depth level. Ice built up inside the tank. Ice in the bot
2689164 Primary Facility 2018-01-19 Incident- Response to Accident or Event No Violations Noted On January 19, 2018, the Department conducted an incident response inspection at the Repsol Oil and Gas USA Inc., Hemlock Valley Well Pad in Pike Township, Bradford County. Andrew Hobbins, Water Quality Specialist, and Jonathan Sassi, Environmental Protection Specialist, represented the Department. Sheridan Anderson, Company Man, was on site for Repsol. The Department was notified of a spill onto secondary containment by Repsol.

The Department was given a tour of the site and the spill location was observed. The spill occurred because the blender shut off, causing the tub to overflow 42 gallons of frac fluid. The fluid was captured by secondary containment and was immediately removed from containment and returned to the work stream. The area could not be inspected more closely due to pressure testing that restricted access to the spill area.

The Department observed the containment liners to be in good condition. No holes were observed in the liner. Daily containment inspections are conducted in

2690802 Primary Facility 2018-01-31 Incident- Response to Accident or Event No Violations Noted I arrived at the site at 11:47 in response to (3) separate spill incidents, a spill of (10) gallons of antifreeze, (2) barrels of flowback fluid, both of these spills to secondary containment, and (2) quarts of power steering fluid to the pad surface. The weather conditions were partly cloudy skies, breezy and quite cold with temperatures around 20 degrees Fahrenheit. I initially met with Dave Coxhead, site safety man of REM security as I arrived. Mr. Coxhead and I then proceeded into the company man trailer to discuss the spill incidents with Sheridan Anderson, site company man/completions consultant of Repsol. Mr. Anderson provided the following information pertaining to the spill incidents and site activities. "Frac is still in progress, but will conclude shortly. We are on stage 19 on the 3H well. There are (3) more stages to frac on wells 1 and 3. We may start drilling out hydraulic fracturing plugs this Monday. The main containment which is the New Pig liner, will stay in place. 2703801 Primary Facility 2018-02-21 Routine/Complete Inspection No Violations Noted I arrived at the site at 12:00. I was accompanied by Andrew Hobbins, DEP O&G WQS out of the Northcentral Regional Office in Williamsport, PA. The weather conditions were mostly cloudy skies, breezy and very mild with temperatures in the 60's. A service (work-over) rig was at the site in the process of drilling out hydraulic fracturing (stimulation) plugs. We met with Rich Cashen, site company man, completions consultant of Repsol. Mr. Cashen informed the Department the following regarding site activities. "We started drilling out frac plugs about (3) weeks ago just after the frac job finished. Basic Energy Services Rig #1562 is currently drilling out frac plugs on the 5H well. The plugs have been drilled out on the 7H-10H wells. There are (32) plugs to be drilled out on the 5H well. Flowback fluid from the drill outs is transferred into frac tanks, not the DYNA tank. The fluid in the frac tanks is taken to a pad where the frac will take place. Gel is used to drill out the plugs. 2748076 Primary Facility 2018-06-19 Routine/Complete Inspection No Violations Noted I arrived at the site at 12:35 PM. The weather conditions are partly cloudy skies and breezy with temperatures in the mid 70's. The Hemlock Valley 1H-10H wells are in production. The secondary containment used for completions operations has been removed from the site. This includes the containment for the DYNA tanks. I met with Jeff Blanton, well tender of Repsol, a short time after speaking with the truck driver. Mr. Blanton informed me that (1,200) barrels of fluid per day flowed from the wells into the production fluid tanks and frac tank when the wells were initially placed into production status, which was about (2) weeks ago. The current rate is about (700) barrels per day he further stated.

The main intent of the inspection was to evaluate the condition of the pad surface where the secondary containment liners were set in place during completions operations. 2768508 Primary Facility 2018-08-24 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection on the Repsol Hemlock Valley (05-265) well pad in Pike township, Bradford County on 8/24/2018 at 1400 hours. The weather was partly cloudy, sunny, and 78 degrees.

The site appeared very well vegetated and stabilized. All post construction best management practices have been properly installed. A well site restoration report has been received and accepted by the DEP

The Notice of Termination for this site is recommended for approval.
