Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2670517 Primary Facility 2017-12-12 Drilling/Alteration No Violations Noted Drilling of the well cellars underway. Drill cuttings being disposed of in roll off container. Drilling performed by Halliburton. No violations noted. Location snow covered.
2696142 Primary Facility 2018-02-20 Drilling/Alteration No Violations Noted Arrived @ 12:00. Weather: warm, T¿s in 40¿s. Precision 569 on site and beginning surface hole drilling on the L. Kingsley SE 4H during my inspection. Met with Ty Merreighn (company man), also onsite were Trey Villaneuve and Paul Ricco (drilling co-superintendent). Chief is employing batch drilling on this location and using the FMC DTO 30 wellhead system. Halliburton conducted the surface casing cement job on the SE2H during my inspection. The surface casing for the SE 3H had been previously cemented.
2699939 Primary Facility 2018-02-28 Drilling/Alteration No Violations Noted Arrived @ 12:00. Weather: warm, overcast. Precision 569 on site and drilling intermediate hole on the L. Kingsley SW 1H. 8 surface holes were finished and 3 intermediate holes cased. Met with Jeremy Boyd (company man). Reviewed 3 surface casing CBLs which showed no abnormalities. Discussed temperature of cement water. Chief is employing batch drilling on this location and using the FMC DTO 30 wellhead system. The order of work is to drill and case, then move the rig and start drilling the adjacent hole while cementing the just-cased wellbore.
2706773 Primary Facility 2018-03-15 Routine/Complete Inspection No Violations Noted I met Ty Marine when I arrived. Precision rig #569 on location, drilling at 6231' on the SW 2H well. 1-3" of snow on the site; snow on rig mats had been shoveled into piles on containment. Overall, site looked good, with containment berms in good condition and good housekeeping. Chemical storage area was neat and clean. I did note 4 vac boxes that had been set off of containment; a small (8' x 4') duck pond had been placed under the end that opens, but these had been buckled and folded under the vac boxes. When I mentioned it to Ty, he said that the boxes were empty, but that he had already called the company that had dropped them off, and they would be returning on Friday to re-set them.
2715347 Primary Facility 2018-03-29 Drilling/Alteration No Violations Noted Arrived @ 10:43 with Ulyana Trynovich (DEP OGI)Weather: overcast, foggy. Precision 569 on site at 3000' on the SE 3H. Met with Jeremy Boyd (company man). Reviewed several intermediate casing CBLs. There appeared to be decreased bond quality at approximately 750', based on a quick review. Requested electronic copies from Peg Farrell (Chief) for further review. Cement sample cups did not appear to be labelled. We agreed to label them consistently. Reviewed BOP test conducted on 3/21/18. Discussed level of detail in drillers log and noted that vertical section was logged. NW 2H from 100' to 1521', and SW1H from 1546' to 8640' MD. Also discussed changes in CBL timing. No violations were observed during this inspection.
2732775 Primary Facility 2018-05-22 Drilling/Alteration No Violations Noted Arrived @ 10:50. Weather: rain. Precision 569 on site at 16663' on the NE3H. Met with Jeremy Boyd (company man). Operations going well. Should skid over to last well on Friday. No violations were observed during this inspection.
2769767 Primary Facility 2018-08-31 Routine/Complete Inspection No Violations Noted Eight wells drilled in track and ready for production. Eight GPU installed within containment. The well supply and dump lines are being installed at this time. Heavy equipment onsite digging trenches for the pipe installation.
2811985 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted I met Greg Rohrer, Chief company man, when I arrived. Flowback operations were in progress, with temporary sand separators and GPUs hooked up and gas flowing to sales. Overall containment appeared to be in good condition, with the exception of the foam berm between the permanent GPUs and temporary ones; in what was obviously a high traffic area, the berm had been crushed. The containment volume for this area may still have been more than adequate, as only pass-through equipment was located on the containment, but I pointed it out to Greg anyway. I noted a 10'x12' puddle on the pad that showed slightly elevated conductance (2.88mS/cm) between the temporary GPUs and the Baker tank battery containment. A vac truck was pumping out the Baker tanks in this area, and Greg told me he would have the area vacced ASAP.
