2661409 Primary Facility 2017-11-09 Routine/Complete Inspection No Violations Noted Company man for site was Trey Villareal, although I did not see or speak to him while on-site. H&P rig 513 was running casing in the 3H well. Containments appeared to be in good condition, and both the cuttings backyard and the chemical containment area were clean.
2687420 Primary Facility 2018-01-23 Routine/Complete Inspection No Violations Noted I met Gibb Blevins when I arrived onsite. Frac is completed, currently moving equipment from the site. Steady rain falling. Puddles on the pad surface showed conductance readings from .35 mS/cm to .85 mS/cm, and most of the water on containment showed similar values. One area of water on the liner (approximately 20' x 80') showed elevated conductance (2.1- 16.3 mS/cm); I pointed this area out to Gibb. Several vac trucks were actively working on the site, and Gibb told me he would have them focus in that area.
2786389 Primary Facility 2018-10-03 Routine/Complete Inspection No Violations Noted Weather was 74° and sunny at time of inspection. There were no Chief representatives on location at time of inspection. Well had a 0-5k psi gauge on the production tubing, the production casing and the pay line. Well also had a digital gauge on the production casing. The digital gauge on the production casing read 1301 psi, tubing pressure was approximately 1500 psi and pay line pressure was approximately 1400 psi at time of inspection. There were 2 annulus vents plumbed to surface. They were plumbed together and ran over to a poly vent tank. The tank had 100% LEL and 60% methane detected at time of inspection. No violations were noted at time of inspection.
2811959 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted I met Trey Villareal, Jeremy Boyd, and Billy Swanson when I arrived. Precision rig 569 was on location, drilling the surface hole on the East 1H well. Containments appeared to be in excellent condition, site housekeeping was good.
2842962 Primary Facility 2019-02-19 Incident- Response to Accident or Event No Violations Noted On February 19, 2019, the Department conducted an investigation on the Chief Oil & Gas LLC Raimo Pad. A split hose caused a spill of base oil outside of containment off the well pad. The operator estimates about 1 bbl of base oil had spilled. At the time of inspection, the soil of the impacted area has been excavated. The soil has been placed in covered roll-off containers. The excavated area did not appear to have any visually impacted soil remaining. The Department spoke with Jeremy Watson with Chief Oil & Gas LLC. He said that Waste Management was coming to do a profile of the contaminated soil and haul it away.

During the investigation, the operator was cleaning and removing timber mats. There was a vac truck onsite to remove the wash water out of the spill containment.

The Department spoke with Peg Farrell on the phone. The Department discussed that the Act 2 process should be started.

The release of base oil of the well pad is a violation of CSL 402(b), 78a 56(a), and SWMA 301.
