All substances within secondary containment. No leaks or spills detected. No violations. Precision Rig 569 on site.
2649786 Primary Facility 2017-10-18 Incident- Response to Accident or Event No Violations Noted I arrived at the site in response to a spill notification received Tuesday 10/17 at 16:00 hrs. Chief reported that approximately 1 barrel of fracwater/brine with friction reducer was spilled to the pad when a valve cover failed on the missile. I met Justin Myers and Chris Marsh when I was on-site. We looked at the area where the spill had occurred, and they informed me that Kellon Balum of Chief arrived soon after the spill and stopped the frac. They observed fluid coming out from under the liner, so Kellon had the equipment removed from the area, the containment pulled back, and immediately began removing the affected soil. Chris estimated that 2.5-3 cubic yards of soil had been removed, and pointed out the pile of soil on containment. Kellon assisted the excavation by screening the soil for TDS and had the excavation continue until the soil reached background levels (100-200 ppm). Once he determined that the release had been removed, he noted several holes in the liner that had been pulled back. Th
2660021 Primary Facility 2017-11-09 Routine/Complete Inspection No Violations Noted I met Floyd Roberts of Chief when I arrived. Workover rig on location to drill out plugs. Some small puddles were present in the area of the spill reported on 10/17; conductivity of these puddles was low (from .098 to .9 mS/cm). Some of the foam berms had been removed in antraffic area on the containment; although I didn't note any chemicals or waste in this area, I recommended to Floyd that he assess whether or not the berms should be put back in place.
2676433 Primary Facility 2018-01-02 Incident- Response to Accident or Event Pending Well is shut in due to a fire in one of the GPU units on 12/29/2017.

The GPU for the 2H and 3H wells were removed from the well pad as well as containment, to allow access to further investigate the fire.

All wells on the pad were temp shut in at time of inspection.

All annulus vents are vented to polly tank on location.
