2717032 Primary Facility 2018-03-15 Routine/Complete Inspection No Violations Noted Arrived at 11:00 with Jon Hafer to conduct inspection for regulatory inactive well status extension request. This is the second extension request for this well. The 9-5/8" x 5-1/2" annulus was open and vented to a 550- gallon polytank where no methane was detected. The 20" x 13-3/8" and 13-3/8" x 9-5/8" annuli could not be observed because the cellar was filled with gravel. Requested a follow- up inspection with the cellar vacced out to observe these annuli. The three other wells onsite were in production. No violations were observed at the time of this inspection.
2789052 Primary Facility 2018-10-12 Routine/Complete Inspection No Violations Noted Three wells are staged for production. Well cellars are filled with pea gravel. Three GPUs are present within felt and plastic containment. Three steel produced water tanks are present and in good condition. Steel containment with spray-in liner is present and in good condition. Approximately one foot of water are present within containment. The conductivity here was 29.9 uS/cm.

The majority of the pad surface has been restored and is well-vegetated with greater than 70% vegetation. A gravel path exists to the well heads, produced water tanks, and GPUs. Temporary BMPs have been removed. Pad side slopes and topsoil stockpile are well-vegetated with greater than 70% vegetation as well.

Site is stable.
