2271764 Primary Facility 2014-04-29 Routine/Complete Inspection No Violations Noted Highlands Drilling Rig #8 is the active drilling rig at the location. The current drilling depth for the Kaufmann Unit 3H well is (1,651) feet. Manny Olmos, site company man and consultant of Chief Oil and Gas accompanied me during the inspection. According to Mr. Olmos, the intermediate string is being drilled through via air/mist drilling and Highlands #8 will drill the well to kick off point, which is at (7,500) feet. The 1H well is in the production phase of operations. A request for an approval of alternative waste management practice (OG-71) application needs to be submitted to the Department in relation to the drill cuttings processing and drilling mud dewatering at this location. The request for an OG-71 approval encompasses both the use of centrifuges for separation of solid and liquid material in the drill cuttings during air/mist drilling and oil based mud drilling and the injection of the chemicals mentioned above used for coagulation and flocculation purposes during air/mist drilling.

The 2H and 3H were in production. The 1H was shut-in at the time of my inspection, but a gas-buster and sand separator had been plumbed into the 1H.

Vegetative cover on site is greater than 70%, and consists of perennial plant species. Temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

The well pad surface has been de-compacted and vegetated, which constitutes interim restoration. The Erosion and Sediment Control General Permit for the site expired on 01/26/2016. The notice of termination for this site was previously approved by the Department on 07/21/2016 (Inspection No. 2503542).

Interim restoration has been implemented on the well pad. A significant portion of the pad surface has been decompacted and vegetated. A total of 3,938 tons of drill cuttings, 1,520 barrels of drilling fluid, and 12,259 barrels fracing fluid are reported to have been disposed of at appropriate waste disposal facilities.

The well site restoration reports are acknowledged.

Annuli plumbed to 550-gallon poly tank. 5% LEL at tank mouth. No violations were observed during this inspection.

2725425 Primary Facility 2018-05-02 Routine/Complete Inspection No Violations Noted The 4H and 5H wells were spudded on 4/18/2018. Conductors set. The 1H, 2H and 3H wells are staged for production. A soil stockpile was installed as shown on site plans. Soil stabilized with straw. It is unknown if fertilizer and seed mixture was applied. Silt stock applied around stockpile. No leaks from production equipment.

2831470 Primary Facility 2019-01-03 Routine/Complete Inspection No Violations Noted I arrived at the site at 12:05. The weather conditions are cloudy skies, chilly, with occasional snow flurries, and temperatures in the mid 30's. A vacuum truck is on site in the process of removing storm water on the pad surface adjacent to the production fluid tanks containment unit. There was no other activity or personnel at the site. The Kaufmann 1H-5H wells are in production. Chief submitted a stimulation notification to the Department on 11/29/18 at 8:58 AM. The comments in the notification indicate the frac started on the 4H well at 7:28 AM on 11/29/18. The 4H and 5H wells are the most recently completed wells. Completions operations consists of hydraulic fracturing (stimulation), hydraulic fracturing plug drill-outs, and flowback. The secondary containment liner and equipment utilized for completions, along with frac tanks, remains on the pad surface. Impacted material associated with a spill of fluid out of a frac blender tub remains on the pad surface in the northeast quadrant of the pad
