2572637 Primary Facility 2017-03-17 Routine/Complete Inspection No Violations Noted On March 8, 2017, the Department conducted a routine inspection at the SWN Production Company (SWN) GU-14-Pad in Herrick Township, Bradford County. The Department was represented by Andrew Hobbins, Water Quality Specialist. Nobody was present for SWN.

The E&S plan, site sign and weekly inspection logs were available for review. The slopes of the well pad were well vegetated. No erosion issues were observed. A Sigma 3 truck was on site conducting seismic testing as part of the completions work being conducted at the GU-4-Pad. The rig was on containment and a vac truck was on site removing stormwater from the containment. Other trucks and equipment were being stored on site as part of the completions work on the 4 Pad. The trucks were parked over containment. There was no staining on the well pad surface. All puddles on the pad were low in specific conductivity.

There was one well drilled on the well pad. No production tanks or GPUs were observed on site. The drilled well was granted inactive

2628733 Primary Facility 2017-08-15 Routine/Complete Inspection No Violations Noted On August 15, 2017, the Department conducted a routine inspection at the SWN Production Company LLC, GU-14-Pad in Herrick Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for SWN.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was well vegetated and no erosion issues were observed. PCSM has been addressed at the site.

One well was in production during the inspection. The well was labeled and no bubbling was observed in the well cellar. One production tank and one GPU was observed in secondary containment. There were 23 frac tanks on secondary containment during the inspection. All of the tanks appeared to be empty. In addition, six frac tanks were not observed on secondary containment, and also appeared to be empty. Puddles on the pad were low in specific conductivity. No staining was observed on the pad surface. The Department did observed several areas of litter present

2707179 Primary Facility 2018-03-15 Routine/Complete Inspection No Violations Noted On March 15, 2018, the Department conducted a routine inspection at the SWN Production Company LLC, GU-Whipple-14-Pad in Herrick Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for SWN.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The last inspection conducted by SWN occurred on 3/13/2018. The site was mostly snow covered. PCSM has been addressed with the installation of a rock infiltration trench. The well pad has not been reduced in size.

One well was in production during the inspection. The well was tagged and no bubbling was observed in the well cellar. One production tank and one GPU were observed in secondary containment. Puddles on the pad tested low for specific conductivity. No staining was observed on the pad surface.

No violations were observed during the inspection.

2816351 Primary Facility 2018-12-17 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of SWN's Whipple 14 Pad in Herrick Twp, Bradford County on 12/17/2018 at 1210 hours. The weather was cloudy and windy. The inspection was conducted in response to a well site restoration report received by the DEP.

There is currently one well in production with one production tank onsite. The site is well vegetated and appears to be stabilized. All PCSM BMPs are in place and appear to be functioning properly. No temporary E&S BMPs were observed onsite.

This well site restoration report is accepted at this time. A Notice of Termination for this site's ESCGP will also be accepted upon submission.

No violations noted today, I departed the site at 1240 hours.
