2786212 Primary Facility 2018-10-09 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected On 10/9/18, the Department conducted an inspection in response to a spill of production fluid at the Chief Wright A Well Pad. The spill occurred on 10/8/18 at 1630. The spill was caused by back pressure from the production tank through a failed hose. Approximately 10 gallons of fluid was spilled.

When the Department arrived on site the impacted area had already been excavated. There was no evidence of the fluid leaving the pad.

The release of production fluids outside of containment is a violation of the following: CSL 402(b), 78a.57(a), SWMA 301. However, since the release had already been remediated in accordance with 78.66(c)(1), the violations are considered immediately corrected.
