Findings: Spill did occur on containment and completely cleaned up. Rec'd report from company foreman of occurrence. Return hose on hydraulic system blew causing release of approx 137 gals of hydraulic fluid. Snubbing unit on location performing pressure tests when incident occurred. This is the Alderson 2H pad (05-011) Permit number 015-21940.
2117638 Primary Facility 2012-08-21 Routine/Complete Inspection No Violations Noted Well drilled, however currently shut in. No gagues on annuli. Asked Talisman rep to repaint numbers on wellheads to differentiate until wells are completed, then permanent well signs will be required.

2667789 Primary Facility 2017-11-29 Routine/Complete Inspection No Violations Noted On November 29, 2017, the Department conducted a routine inspection at the Repsol Oil & Gas USA LLC (Repsol) Alderson V 05-011 well pad in Pike Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Repsol.

The site sign, E & S plans and weekly inspection logs were on site and available for review. The site was well vegetated. The well pad has not been reduced, but PCSM has been addressed at the site.

Four wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellar. Two production tanks and four GPUs were on secondary containment. A closed top roll-off container was also on secondary containment. The contents of the roll-off container are unknown, but the container was labeled as "residual waste." Puddles on the well pad surface were low in specific conductivity. No staining was observed on the pad.

No violations were observed during this inspection.
