2651118 Primary Facility 2017-10-18 Routine/Complete Inspection No Violations Noted On October 18, 2017, the Department conducted a routine inspection at the Repsol Oil and Gas USA LLC, Slovak M (05-202) well site in Windham Township, Bradford County. The Department was represented by Andrew Hobbins, Water Quality Specialist. Nobody was present for Repsol.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The site was mostly well vegetated, but patches of dead vegetation were observed on the eastern and southern slopes. The Department recommends temporarily stabilizing and reseeding the areas. No major erosion or sedimentation issues were observed. PCSM has been addressed at the site. Two bioretention basins were observed to be installed. Bioretention basin #2 had an uncapped HDPE outlet pipe. The ESCGP plans call for a capped 4" HDPE outlet pipe. The Department recommends capping the outlet pipe.

One well was in production during the inspection. The well was labeled and no bubbling was observed in the well cellar. One GPU and two p
