Met with company man, Pat Bauer; night man, Trent Anderson; and rig move supervisor Gary Reich. Drilling the 3H went smoothly according to Bauer. TD was 12990’ and cementing went very well. After drilling the 5H the rig will drill the 6H at this location and then move to the Bennett location. BOPs were last tested on 3/17/15. During my inspection rock was being laid down to address the muddy condition. Left location @ 12:30.

2482528 Primary Facility 2016-05-09 Incident- Response to Accident or Event No Violations Noted Sheridan Anderson, Completions Company Man of Talisman Energy contacted the Department on 5/8/16 to report a spill of fluid to secondary containment. The Pennsylvania Department of Environmental Protection Standard Detail Report lists the following information in the long description of the spill. Sheridan Anderson of Talisman Energy contacted Phone Duty Officer (Anthony Liguori) that approx. 10:00 a.m. on 5/8/16 during well #6 frac, there was a mechanical failure of the pop-off valve, spilling an estimated 5 barrels of fluid on secondary containment and 1 barrel on primary containment. The spill was vacuumed up at time of call. Anderson noted using 100% fresh water, though high salinity was observed, possibly from intermingling with flowback from other wells on site.

I arrived at the site at 12:06. Halliburton is the Talisman subcontractor conducting hydraulic fracturing operations. Talisman submitted stimulation notifications to the Department on 4/14/16. This notice stated that frac operations for t

2485579 Primary Facility 2016-04-29 Routine/Complete Inspection No Violations Noted I conducted an inspection of the site accompanied by Andrew Hobbins, DEP O&G WQS. As we arrived at 10:30, Talisman was in the process of setting up for hydraulic fracturing operations. Talisman submitted a Stimulation Notice to the Department on 4/14/16 indicating that hydraulic fracturing operations will commence on 5/5/16 at 8 AM. We met with Billy Watts, site company man of Talisman Energy. Mr. Watts stated the following information. "Talisman commenced setting up for frac operations on 4/21/16. There are (65) frac tanks stored in the tank battery on containment next to the fill slope, full of production fluid which is transported to the site via vacuum trucks from other Talisman Pads. All frac tanks and gas buster tanks on-site have 110% containment capacity. The above ground storage tank contains around (1) million gallons of freshwater. The water is pumped through an underground pipeline from a nearby Talisman freshwater impoundment. There is a (100) mil in capacity New Pig Liner set in place bene
2492837 Primary Facility 2016-06-13 Incident- Response to Accident or Event No Violations Noted David Williams, Water Management Coordinator of Talisman Energy contacted the Department at 11:25 hours on 6/12/16 to report a production fluid spill. The PADEP Emergency response notification report states the following information. The spill occurred between 5:00 and 6:00 AM on 6/12/16. Production fluid left in the drip pan of water truck being loaded, spilled onto multiple sections of road during transport. Loading of the truck began at Upham well site by Fast Trucking. Production Fluid spilled from the drip pan at the turn out/entry to pad, at the intersection of State Route 467 and & CC Allis Road by state barn, and the intersection of Wolf Hill Road and South Hill Road. Mr. Williams reported that the affected areas were swept/vacuumed and loose stone was collected. Talisman intends to test and landfill clean up materials. There is no sign of runoff to wetlands or waterways according to Mr. Williams.

2493348 Primary Facility 2016-06-07 Routine/Complete Inspection No Violations Noted On June 7 2016 at 1100 hrs., the Department conducted a routine inspection at the Talisman Energy USA Inc.(Talisman), Upham (05-129) well site in Pike Township, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Dan Machaceck was the foreman on site for Talisman.

The Department walked around the well pad taking specific conductivity readings from puddles on and off containment. An area of slightly elevated conductivity was observed on the well pad between the 4H well and the GPU’s. The readings from the puddle were around 1600 µs/cm. All other conductivity readings were below 1000µS/cm. The Department recommends removing the liquid from the pad surface. Mr. Machaceck stated that they have been using vac trucks to remove rainwater from containment structures and have been putting the water back into the tank battery on-site.

The containment system appeared to be in good condition during the inspection. No staining was observed on the well pad surface. A

2509460 Primary Facility 2016-07-15 Routine/Complete Inspection No Violations Noted Kris Perritt, Environmental Specialist of Talisman Energy forwarded an email to the Department on 7-14-16 containing a post frac conductivity assessment report consisting of Upham Post Completions EC Screening Data Summary 07-12-2016. The post frac assessment consists of electrical conductivity screening and flowback water remediation activities. The report also contains an electrical conductivity contour and proposed excavation location map. Golder Associates performed post hydraulic fracturing conductivity assessment of the pad surface by spraying distilled water on the pad surface and utilizing a soil conductivity probe immediately after spraying the water. There are (273) locations of the pad surface where soil conductivity readings were recorded. The highest reading recorded was at Point ID #94, was (15,560) microSiemens/cm³.
