Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2007962 Primary Facility 2011-10-04 Routine/Complete Inspection No Violations Noted
2264319 Primary Facility 2014-04-01 Site Restoration No Violations Noted Restored pad, approx. 91m perimeter. 1 well drilled, not in production.
2637590 Primary Facility 2017-09-19 Routine/Complete Inspection No Violations Noted Inspection to assess overall site conditions. The well sit is restored. No violations cited.
2680857 Primary Facility 2018-01-10 Routine/Complete Inspection No Violations Noted Well is shut in at time of inspection.

Well is on inactive status at time of inspection.

All anulus vents are connected to one 1 1/4 inch vent vented to polly tank on location, tested for LEL & Methane at polly tank with an altair 5x multigas detector. Readings were 100% LEL and 82% Methane/Vol at time of inspection, was unable to specifically identify what annulus vent was venting due to all vents being vented to a single vent line.

2,000 PSI on the production tubing.

2723648 Primary Facility 2018-04-26 Routine/Partial Inspection No Violations Noted I took combustible gas readings from the 3 annulus vent barrels (photo) with an Altair 5x meter. The 1st 2 barrels gave 0% LEL & methane readings. The 3rd barrel gave a reading of 11% methane.

I did not take electrical conductivity readings, because I did not see any accessible standing water on site (there is some water in the black tote that holds the 3 blue barrels).

2727448 Primary Facility 2018-05-08 Routine/Complete Inspection No Violations Noted The three annulus vents have been separated into individual vents and are vented into plastic drums. Annular vents are not identifiable at time of inspection.
2735253 Primary Facility 2018-05-21 Routine/Complete Inspection No Violations Noted Reason for inspection is to meet with EOG to collect gas samples from each of the annulus vent and perform A 72 hour PBUT. On location with Caleb Woolever (PADEP), Clayton Schucker (Moody and associates), and Ed Sicora (EOG). PBUT valves closed at 13:18 pm. 2,220 psi production casing.
2801736 Primary Facility 2018-11-13 Routine/Complete Inspection No Violations Noted Cool/snowy at time of inspection.

All vents are vented to plastic 55 gallon drums on location.

Performed headspace test around well head, Test results showed no Methane or LEL present.

Methane/LEL % by volume from all three annulus vents are consistent with prior inspections at this well site.

2828256 Primary Facility 2019-01-17 Routine/Complete Inspection No Violations Noted Cool/windy at time of inspection.

All vents vented into plastic barrels onsite.
