2788422 Primary Facility 2018-10-03 Routine/Complete Inspection No Violations Noted The DEP conducted a RESTORATION inspection of the Respol Oil & Gas's White E (03-025) well pad in Columbia Twp, Bradford County on 10/3/2018 at 1030 hours. The inspection was conducted in response to a Well Site Restoration Report received by the DEP for API # 015-21016. The weather was 57 degrees, 87% humidity and cloudy.

There is currently 1 active well onsite with another 7 wells plugged. There is one production tank onsite with no corrosion observed. The site is very well vegetated and appears to be stabilized. No temporary E&S BMPs were observed and all PCSM practices appear to be in place and functioning properly.

This Well Site Restoration Report is accepted at this time. Due to this site meeting stabilization requirements, I would also recommend terminating this site's ESCGP.

No violations noted at this time. I departed the site at 1100.
