2531216 Primary Facility 2016-10-06 Routine/Complete Inspection No Violations Noted Arrived at 11:30. Weather cool and sunny. This inspection was conducted in conjunction with an Application for Inactive Well Status. There was one well on a small pad. The cellar was filled in and cat tails were growing in close proximity to the well head (see photo). A methanol injection unit was in pieces on location. There was a large green polytank on containment in an area excavated down approximately 1 foot. Solar cells lying on the ground appear to have fallen from a metal mast near the well head. The well was locked and tagged. There was one annular vent, labelled 9 x 5. Field monitoring with an Altair 5X gas meter (calibrated on 10/5/16) obtained 100% combustible gas/volume. The flow appeared to be negligible. Also on the pad were: two produced water tanks and a small GPU in containment. There was also a pipeline connection, located at the edge of the pad. Adjacent to this was a small polytank containing corrosion inhibitor. Contacted Jon Hafer to request a joint follow-up inspection to determine
2531362 Primary Facility 2016-10-25 Routine/Complete Inspection No Violations Noted Arrived at 09:00. Weather: cloudy and cold. This inspection follows up on a previous Inactive Status Application inspection (10/6/16, 2531216). Met with Jon Hafer who confirmed that the well had been previously fracced and had been in production. Confirmed 100% combustible gas/volume on 9-5/8” x 5-1/2” annulus, no measureable flow. He explained that the 13-3/8” x 9-5/8” annulus was underneath the cellar gravel, confirmed that the large green polytank had been used for methanol. The well record listed a total of 1704 sx cement for the production casing which suggested the casing was cemented to surface. Hafer’s information indicated that the production casing cement top had been determined by CBL and was close to the kick-off point. He agreed to look for this CBL. No violations were noted at the time of this inspection. Left location @ 10:16.
2545042 Primary Facility 2016-12-13 Routine/Complete Inspection No Violations Noted Arrived at 11:10. Weather: cold. This inspection follows up on previous Inactive Status Application inspections (10/6/16, IR# 2531216; 10/25/16, IR# 2531362). Met with Jon Hafer (Chief). Also onsite were Ethan and Bob (Mountain Energy) with a vac truck. The 1H is the only well on the pad. It was locked and tagged. The gravel, cat tails and other vegetation present during previous inspections had been removed and 13-3/8” x 9-5/8” annulus was clearly visible. At the start of the inspection the water level was below the surface casing top. No bubbling was observed. When the water level was increased to cover the13-3/8” x 9-5/8” annulus, no bubbling was observed. The 9-5/8” x 5-1/2” annulus whose plumbing to the produced water tank had been blocked, was newly plumbed to the large green polytank previously used for methanol. Equipment and materials not in use had been removed from the site. No violations were noted at the time of this inspection. Left location @ 12:23.
