2132981 Primary Facility 2013-01-23 Routine/Complete Inspection No Violations Noted E&S plan reviewed as a result of an OG-57 waiver application submitted by the operator for the proximity of the access road to the 100 year floodplain. An OG-57 is not required for this site because the well-site itself is not within the floodplain nor is the stream a solid blue lined stream as found on the most recent 7 1/2 minute topographical map. The E&S plan as outlined should effectively minimize accelerated erosion and sedimenation provided the plan is properly implemented and maintained.
2137428 Primary Facility 2013-02-04 Routine/Complete Inspection No Violations Noted Subsequent to previous inspection, it was determined that the adjacent stream is depicted with a solid blue line by USGS in on-line maps. Accordingly, site was inspected based on operator's OG-57 waiver request. The operator has re-routed a portion of the access road from previous plans minimizing potential erosion and sediment issues. If the E&S plan is implemented as designed with E&S controls properly installed, no accelerated erosion or off-site transport of sediment is anticipated.
2146218 Primary Facility 2013-02-27 Routine/Complete Inspection No Violations Noted At the time of this pre-operational inspection all perimeter E&S controls appeared to be properly installed. The on-site E&S plan appeared to be adequate. No erosion issues were evident.

Site and access road controls had been implemented properly and there was no observed evidence of accelerated erosion or sedimentation in relation to the site or access road. Drill pit located onsite and it contained drill cuttings and fluids. Pit had more than the required two feet of lined freeboard. It was noted that the diesel tank located onsite had the neccesary secondary containment.

Brine tanks located onsite and they appeared to be in sound condition. Drill pit pit had been reclaimed. Tanker truck was onsite to empty the two baker tanks located onsite. There was no evidence of accelerated erosion or sedimentation related to the site or access road at the time of the inspection. The Department recommends that the operator inspect the access road and site erosion controls afte the recent strom activity in the area to insure that the controls do not require any repairs or maintenance.
2196972 Primary Facility 2013-08-13 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed. Well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. There was no evidence of accelerated erosion related to the access road at the time of the inspection. There was a minor erosion issue noted in the fill slope after the outlet of the collection ditch. Specfically runoff conveyed from the ditch had begun to erode a channel in the fill slope up to the silt sock perimeter E&S control. The Department recommends that the operator address this issue as soon as possible so that it does not develop into a more serious erosion issue.
2231752 Primary Facility 2013-12-23 Site Restoration No Violations Noted Well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. Site had been effectively restored and the vegetative cover met the 70% uniform coverage criteria. There was no observed accelerated erosion or sedimentation in relation to the site or access road.
2306102 Primary Facility 2014-09-23 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the site or access road. Vegetative cover met the 70% uniform coverage.
