Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2106470 Primary Facility 2012-10-10 Routine/Complete Inspection No Violations Noted Site constructed, drilling had not commenced at the time of the inspection. Access road and site E&S comtrols had been implemented as outlined in the onsite E&S plan. Drill pit constructed and properly lined. The E&S controls as implemented should effectively minimize accelerated erosion or sedimentation, provided the controls are properly maintained.
2111672 Primary Facility 2012-10-31 Routine/Complete Inspection No Violations Noted At the time of inspection the drill rig was onsite and in the process of setting up to start drilling The Erosion and Sedimentation plan was onsite and all BMPs appear to implemented and maintained as outline in the E&S plan. No observation of accelerated erosion and sedimentation in relation to the access road or well pad area at the time of inspection. The drill pit located onsite appears to be in sound condition with the required 2 feet of freeboard. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure site stability and BMPs are maintained.
2116620 Primary Facility 2012-11-19 Routine/Complete Inspection No Violations Noted Drilling had been completed at time of inspection, but not yet equipped for production. Drill pit located onsite and it contained both fluids and drill cuttings. Drill pit had more than the required two feet of line freeboard. E&S control BMPs properly maintained and no accelerated erosion or sedimentation issues were observed in relation to the well pad and access road. It was noted that the rock construction entrance would require restablishment.
2123196 Primary Facility 2012-12-13 Routine/Complete Inspection No Violations Noted Drilling had been completed at time of inspection, but not yet equipped for production. Drill pit located onsite and it contained both fluids and drill cuttings. Drill pit had more than the required two feet of line freeboard. E&S control BMPs properly maintained and no accelerated erosion or sedimentation issues were observed in relation to the well pad and access road. It was noted that the rock construction entrance would require restablishment prior to commencement of well stimulation activities.
2127654 Primary Facility 2013-01-02 Routine/Complete Inspection No Violations Noted Well had been drilled but not equipped for production. Site E&S controls had been implemented as outlined in the onsite E&S plan. Earth-moving equipment was active on site during the inspection, probably preparing the site for arrival of stimulation equipment. A rock pile was noted at the top of the access road apparently to be used to re-establish the rock-lined entryway as recommended in the previous inspection.
2142582 Primary Facility 2013-02-20 Drilling/Alteration No Violations Noted Well shut in on 4 1/2” x 2 swage. Waiting on frac. Pit lined and freeboard maintained.
2160863 Primary Facility 2012-04-09 Routine/Complete Inspection No Violations Noted Site was drilled but not equipped for production. Pit had the necessary 2 feet of freeboard. E & S controls appeared adequate. No E & S issues were noted.
2173590 Primary Facility 2013-06-07 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed. Site and access road erosion controls appeared to be functioning properly and there was no eviedence of accelerated erosion or sedimentation. Drill pit located onsite and it contained fluids, drill cuttings and it had more than the required two feet of lined freeboard.
2340601 Primary Facility 2015-02-03 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. There was no observed accelerated erosion or sedimentation in relation to the site or access road. Brine tanks located onsite and they appeared to be in sound condition.
2443616 Primary Facility 2016-01-26 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. There was no observed accelerated erosion or sedimentation in relation to the site or access road. Brine tanks located onsite and they appeared to be in sound condition. Site had been properly restored and stabilized with vegetation attaining 70% uniform coverage.
2739496 Primary Facility 2018-06-11 Routine/Complete Inspection No Violations Noted This well is tagged and in production. The annulus is open. The well head casing, piping, production set equipment and two grounded 100 bbl fiberglass fluid collection tanks look ok.
2844013 Primary Facility 2019-02-25 Routine/Complete Inspection No Violations Noted On 2/25/19 at 1248 hrs., I conducted a routine inspection of Snyder Brothers, Inc.'s Heffelinger-Croyle Unit 1M well (permit 005-31059) in Washington Twp., Armstrong Co. The well pad is restored and permanently stabilized. The well is properly labeled and equipped for production. A well tag is present at the well head. A site ID sign is posted at the entrance to the well pad. There are no violations noted for this inspection.
