Spoke with operator representative via phone regarding the addition of rock to the outflow area of the sediment trap implemented at te base of the fill slope. There was no observed accelerated erosion or sedimentation in relation to the access road or well-site.
2063950 Primary Facility 2012-05-01 Routine/Complete Inspection No Violations Noted Inspection performed as a result of a resumption of drilling notice. Drilling had not as of yet re-commenced. Erosion controls had been implemented and maintained properly. There was no accelerated erosion or sedimenation observed in relation to the access road or well-site. Drill pit constructed onsite and it contained minimal fluids attributed to precipitation. Operator should continue to maintain the site and access road until well development activities are completed.

There was no evidence observed of accelerated erosion or sedimentation related to the access road or well-site. It was noted that the sediment trap implemented at the outlets of the collection ditches required routine maintenance to insure its continued functionality.

There was no observed accelerated erosion or sedimentation in relation to the site or access road. It was noted that the sediment trap implemented at the base of the fill slope for the well pad had been left in place. The operator is reminded that they are responsible for continued maitenance of this erosion control. Well appeared to be equipped for production and brine tanks were located onsite. Brine tanks appeared to be in sound condition.

There was no observed accelerated erosion or sedimentation in relation to the site or access road. Well appeared to be equipped for production and brine tanks were located onsite. Brine tanks appeared to be in sound condition. The sediment trap that had been at the base of the well-site had been removed and that area had been regraded. The site had been seeded and mulched and the vegetation was beginning to take hold. The operator should continue to monitor the site for erosion where the sediment trap had been and to insure that 70% uniform vegetative cover had been achieved.

2836128 Primary Facility 2019-02-04 Routine/Complete Inspection No Violations Noted On 2/4/19 at 1343 hrs., I conducted a routine inspection of MDS Energy Development, LLC's Ralph Myers etux 2M-28 well (API 005-31024) in Gilpin Twp., Armstrong Co. The well pad is restored and permanently stabilized. The well is equipped for production. A site ID sign is present at the entrance to the well pad. I was unable to access the property to determine if a well tag was affixed to the well head due to a locked gate. I will contact he operator at a later date to arrange access to the site. There are no violations noted for this inspection.
