Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1973864 Primary Facility 2011-05-17 Routine/Complete Inspection No Violations Noted
1976087 Primary Facility 2011-05-13 Routine/Complete Inspection No Violations Noted Well-site not constructed , drilling had not commenced. Spoke with operator representative Walt Emquist and the drill cuttings will be landfarmed and the pit will only be utilized during the dry times of the year due to the probable depth of the seasonal high groundwater table. The land farmed area must be at least 100 feet from the stream according to 78.63. The culvert placed adjacnet to the well-site should have effective erosion controls (extended culvert or rock lined ditch) to isolate the drainage and stabilize the disturbed area. The E&S plan with these changes should effectively minimize accelerated erosion and sedimentation provided the plan is implemented and maintained properly.
2147267 Primary Facility 2013-03-12 Routine/Complete Inspection No Violations Noted Drilling had been completed well appeared to be equipped for production. No accelerated erosion or sedimentation was observed in relation to the site or access road.
2435377 Primary Facility 2015-12-22 Routine/Complete Inspection No Violations Noted No tank. Tag and e&s ok.
