1963464 Primary Facility 2011-04-05 Routine/Complete Inspection No Violations Noted Met onsite with Ted Parish of Exco Resources (PA) Inc. and Thomas McCombie of HRI Inc for a pre-site construction meeting. Reviewed the E&S plan form the ESCGP-1 (ESX10-005-0009) for the proposed well pad and impoundments. E&S control BMP¿s should effectively minimize accelerated erosion and sedimentation provided they are implemented and maintained properly. WQS Instructed Mr. Parrish to insure the pad, impoundments and erosion controls are constructed as shown in the approved ESCGP-1 and if any changes are necessary to contact WQS Higgins prior to making or implementing those changes. Made a minor revision to the location of a line of silt sox below the impoundments. The silt sox line was moved approximately 20 feet up the hill above a brush line so as to not have to disturb that area by removing the brush to implement the silt sox. This minor change was made and initialed by Thomas McCombie and WQS Gregory Higgins.

Erosion controls implemented as outlined in the ESCGP-1. Observed no accelerated erosion or sedimentation in relation to the site or access road. The operator should continue to monitor the erosion controls to insure their continued functionality until well development activities are completed.
