There were approximately 40 roll-offs boxes sitting on the well pad. Currently the storage of the roll-off boxes containing sludge from the frac flowback pit and the liner from the frac-flowback pit are not on secondary containment. This does not meet the requirements that all hydraulic fracturing additives and flowback be on containment as outlined in the Oil & Gas Act 3218.2(c). On future well pads all roll-off boxes which contain frac flowback sludge and frac flowback impoundment liner should be stored on containment to minimize the possibility of a discharge of liquids from these tanks onto the ground.

2410908 Primary Facility 2015-09-28 Incident- Response to Accident or Event Violation(s) Noted Inspection performed as result of a operator reported spill of production fluids (brine) at the Wright Multi-Well Pad. According to information provided to the Department by the operator the spill ocurred on 09/25/15. According to the report from the operator the spill was the result of a failed vacuum box door and the failure of it's secondary containment. The volume spilled was at most 50 gallons, the operator is quanitifying a specific volume based upon tank measurements. After discovery of the spill the operator undertook the following remedial actions: The affected media was overexcavated to insure all affected media was removed. The area was then backfilled and temporarily stabilized. The affected media was stored in roll-off boxes. The Department recommends that the operator review and apply the Department's Oil & Gas Spill Policy in addition to the Program Clarification memo that outlined which constituents to test for as a result of a spillonce the operator determines the specific volume that was spilled.
2419525 Primary Facility 2015-10-28 Administrative/File Review Outstanding Violations - No Viols Req'd This inspection is to document that the Department has received EQT's response outlining the interim remedial actions undertaken and the decision by EQT to utilize the alternate remediation option outlined in the Department's Policy on Addressing Spills and Releases at Oil & Gas Well Sites or Access Roads. At this time the Department is requesting that by December 28th, 2015 EQT, submit the narrative and sample analysis documentation for the confirmatory and background samples taken as part of the remedial actions related to this spill. This documentation should be sent to the attention of WQS Gregory Higgins at the above listed address.
