1965018 Primary Facility 2011-04-12 Routine/Complete Inspection No Violations Noted Drilling was ongoing at the time of the inspection. Drill pit had more than the required two feet of lined freeboard. Access road and site erosion controls appeared to be functioning properly. Operator should continue to maintain all E&S controls until well development activities are completed.
1965022 Primary Facility 2011-04-12 Routine/Complete Inspection No Violations Noted Drilling had been completed. Well was not equipped for production. The majority of the site had been regraded & mulched, however the topsoil pile was left upslope of the well indicating that final site regrading was as of yet unfinished. Operator should maintain all erosion control BMP's until well development activities are completed.

Drill pit had drill fluids removed and the pit was re-lined and contained what appeared to be frac flowback fluid. The pit had more than the required two feet of lined freeboard. Observed accelerated erosion or sedimentation in relation to the access road or well-site. All erosion controls functioned properly. The operator should endeavor to complete well development and site restoration activities as soon as possible.

Brine tank located onsite and it appeared to be in sound condition. Site had been properly regraded and the vegetative cover met the 70% uniform covera criteria. There was no observed accelerated erosion or sedimentation observed in relation to the access road or well-site.
2110317 Primary Facility 2012-10-24 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition.
