1976397 Primary Facility 2010-09-21 Compliance Evaluation No Violations Noted Inspection performed as a follow-up to a previously issues NOV. At this time the well is drilled and the pit is sound. The well is not equipped for production. There is no visible evidence of the gray drilling mud in or near the stream. There is also no evidence of an impact to the quality of the stream or the aquatic life. At this time, the NOV can be closed.
1980089 Primary Facility 2011-06-10 Routine/Complete Inspection No Violations Noted Inspection performed as a follow-up to a previously issues NOV. At this time the well is drilled and equipped for production.
2089002 Primary Facility 2012-08-02 Site Restoration No Violations Noted At the time of inspection the well appears to be drilled and equipped for production. The well apparatus, brine tank and pump jack located onsite appear to be in sound condition. the site appears to be restored and meets the 70% uniform vegetative coverage criteria. No observation of accelerated erosion and sedimentation observed in relation to the well pad area or the access road.
2213380 Primary Facility 2013-10-16 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. There was no observed accelerated erosion or sedimentation related to the site or access road. It was noted that the access road required rotuine maintenance to insure its continued passability.
2727390 Primary Facility 2018-05-08 Routine/Complete Inspection Pending This well is tagged and in production. The well head casing, piping, well location equipment and grounded 100 bbl plastic fluid collection tank look ok.

NOTE: The small compressor plumbed in to the well(s) in this lease area is in need of some housekeeping. Oil is leaking off of the compressor skid and is beginning to impact the vegetation.

This inspection is written with a potential PENDING Notice Of Violation depending upon the operators response to the compressor site for the wells in this area. I will be placing a phone call to the operator to discuss immediately following this inspection and will conduct a follow up inspection shortly.
