1909905 Primary Facility 2010-09-01 Routine/Complete Inspection No Violations Noted Drill rig located onsite and drilling was ongoing at the time of the inspection. Erosion controls implemented as outlined in the E&S plan. No accelerated erosion & sedimentation was observed in relation to the access road or well-site. Below grade pit located onsite and it had more than the required two feet of lined freeboard. Frac pit located at the Dickey 2H site will likely be utilized for well stimulation at the Dickey 3H.

1928765 Primary Facility 2010-11-17 Routine/Complete Inspection No Violations Noted Drill rig onsite, drilling did not appear to be ongoing during inspection. During a cement job on the morning of 11/17/10 a fitting broke releasing approximately one barrel of a synthetic drill mud to the well pad and adjacent pad liner. In response to this spill the operator began clean-up operations after contacting OGI Supervisor Ed Renwick. At the time of the inspection it was noted that the drill mud had stayed on the surface of the well pad and that the affected media was being gathered and placed with the drill cuttings to be hauled off-site as there is no drill pit onsite. WQS Higgins spoke with Exco representative Doug Byers and requested that he forward via e-mail an outline of when the spill occurred, when cleanup began, what the clean-up consisted of, and when the clean-up was completed. Mr. Byers indicated that this should be compiled and forwarded the morning of 11/18/10. This information was received on 11/18/10 at 11:40 AM.
1929875 Primary Facility 2010-11-02 Drilling/Alteration No Violations Noted drilliing @ 7597ft/E&S ok/ID&pit ok

1930822 Primary Facility 2010-11-24 Follow-up Inspection Violation(s) Noted re-inspection for ESX10-005-0007 for erosion and sed. violations, wetland and stream encroached and damaged, construction of gasline outside of permit area of disturbance on Frederick property, silt fence not maintained, water bar on access road to compressor station not properly constructed, slit fence installed across streams. Advised operator, Perry Lupinetti of EXCO, to restore streams and construct proper stream crossing on Frederick property, need better maintenace on silt fences and repair water bar on access road
1952058 Primary Facility 2011-02-22 Routine/Complete Inspection No Violations Noted Drilling had been completed, wells appeared to be equipped for production at the time of the inspection. Brine tanks located onsite and appeared to be in sound condition. Site had not as of yet been regraded. Erosion controls appeared to be holding up and would continue to function properly.
1957427 Primary Facility 2011-03-15 Routine/Complete Inspection Viol(s) Noted & Immediately Corrected Drilling had been completed, wells appeared to be equipped for production at the time of the inspection. Brine tanks located onsite and appeared to be in sound condition. Site inspection performed as a follow-up to a release of production fluids reported by the operator on 03/11/11. Due to a pipe failure on the production unit for the 3H well a mist of production fluids was released to the immediate area around the 3H and 1H production units. Due to this the affected media was excavated, contained onsite in plastic pending sample results. The affected area was subsequently sampled to insure that all of the affected media had been removed. Visual inspection found that it did not appear that the fluid release had made it past the area excavated immediately surrounding the production units. Documentation from the operator detailing how the release occurred, cleanup activities and actions taken to prevent a reoccurrence is forthcoming after a phone conversation with operator representative Doug Byers.
1974143 Primary Facility 2011-03-31 Routine/Complete Inspection Violation(s) Noted Inspection in response to a methanol spill. Contaminated media had been excavated and contained onsite prior to site inspection.
1975583 Primary Facility 2011-04-28 Compliance Evaluation No Violations Noted Drilling had been completed, wells appeared to be equipped for production at the time of the inspection. Brine tanks located onsite and appeared to be in sound condition. Noted that the excavated material from two recent fluid discharges was still onsite on and covered by plastic. Observed no accelerated erosion or sedimentation in relation to the access road or well-site.
2075386 Primary Facility 2012-06-08 Routine/Complete Inspection No Violations Noted Drilling had been completed, site had been regraded. Well appeared to be equipped for production. Brine tanks located onsite and they are within containment. Containment contained minimal precipiation water which showed no sheen. Noted that the vegetative cover did not meet the 70% uniform coverage criteria. Spoke with EXCO representative Ted Parrish regarding this matter and he stated that he is contacting a contractor today to have the site reseeded as the grass seed drilled with the oat seed had not germinated.
