The erosion controls outlined in the E&S plan and the nature of the site ( a paved lot) should effectively minimize accelerated erosion & sedimentation. Based upon submitted information drill cutting disposal of encapsulation should meet all requirements set forth in 78.62.

Auxillary pit noted as being onsite as a backup should a large quanity of water be encountered during drilling activities. Noted the site was muddy, the Department recommends that the operator monitor the traffic associaeted with well development to insure that sediment generated onsite is not transported to the paved road North Water Street.

1897365 Primary Facility 2010-07-09 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed, well was not equipped for production at the time of the inspection. Below grade pit located onsite and it contained a small amount of fluids and drill cuttings. Pit had more than the required two feet of lined freeboard. Site had not as of yet been regraded. Due to the flat nature of the site minimal erosion controls were required. Should the operator choose to stimulate the well the operator should monitor the site to insure that sediment generated during truck traffic is not transported off-site.
1948629 Primary Facility 2011-02-08 Routine/Complete Inspection No Violations Noted Drilling had been completed, well appeared to be equipped for production. Brine tank located onsite and appeared to be in sound condition. Site had been regrded.
2742887 Primary Facility 2018-06-19 Routine/Complete Inspection No Violations Noted This well is tagged and in production. The annulus is open. The well head casing, piping, equipment and grounded 100 bbl fiberglass fluid collection tank look ok.

NOTE TO OPERATOR: - This well has an E&S paperwork tube posted right beside the well head with a permit copy with the well permit number inside it. This is the only place that the permit number can be found on this well pad. Please place the usual permit number tag on the well head. Thank you.
