1880067 Primary Facility 2010-04-27 Routine/Complete Inspection No Violations Noted Drill rig located onsite and drilling was ongoing at the time of the inspection. Below grade pit located onsite and it had more than the required two feet of lined freeboard. Observed that the silt fencing had been installed properly and there was no accelerated erosion & sedimentation in relation to the site. Noted that the erosion controls shown on the E&S plan for the access road (i.e. culverts and inlet and outlet protection) were not observed as being installed on the access road. In addition muddy Pick-up tire tracks were observed leaving the site onto State Route 66. Due to this fact and the muddy nature of the site at this time the operator should evaluate the access road for the necessity of additional rock and any other measures to be taken to insure the minimization of generation and transportation of sediment from the site.
1890627 Primary Facility 2010-06-10 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed and well appeared to have been stimulated. Well was not equipped for production. Pits located onsite had more than the required two feet of lined freeboard. Observed no accelerated erosion & sedimentation in relation to the access road or well-site. The operator should endeavor to reclaim the site and seed & mulch as soon as possible.
1901637 Primary Facility 2010-07-29 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed and well appeared to have been stimulated. Well was not equipped for production. Pits located onsite had more than the required two feet of lined freeboard. Noted that the below grade pit appeared to have been taking on runoff water laden with sediment. WQS Higgins observed no accelerated erosion or off-site transport of sediment in relation to the access road or well-site. Noted that a small build up of sediment had accumulated at the transition of the access road to the paved road. The operator should endeavor to reclaim the site and seed & mulch as soon as possible.
2072293 Primary Facility 2012-05-30 Routine/Complete Inspection No Violations Noted Drilling completed, well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. No accelerated erosion or sedimentation observed in relation to the access road or well-site. Site had been properly restored.
