1877351 Primary Facility 2010-04-14 Routine/Complete Inspection No Violations Noted Drill rig onsite, drilling had not commenced. Rig crew was in the process of lining the below grade drill pit. Silt fencing backed with hay bales had been properly implemented below the site as an appropriate erosion control BMP. Noted that the culvert implemented adjacent to the well pad should have additional rock placed at the outlet to carry the flow to an undisturbed area and insure the effectiveness of the BMP.
1877842 Primary Facility 2010-04-19 Routine/Complete Inspection No Violations Noted Drill rig onsite, drilling was not ongoing due to an equipment problem. Below grade pit located onsite and it had more than the required two feet of freeboard. Observed no accelerated erosion & sedimentation in relation to the access road or well-site. Noted what appeared to be a relatively large possible wetland area below the site in between an unnamed tributary to Keystone Lake and the well-site. At this time the Department is requesting that the operator evaluate this area to determine the area in question is a wetland and if so the size of the wetland. Documentation outlining the evaluation of the area should be forwarded to the Department no later than May 14, 2010.

1965639 Primary Facility 2011-04-14 Site Restoration No Violations Noted Drilling was completed on 04/22/10. Well appeared to be equipped for production. Brine and oil tanks located onsite and both appeared to be in sound condition. Well-site had been restored and no accelerated erosion & sedimentation was observed in relation to the access road or site. Noted an area around the dike which had been recently seeded & mulched. Once the vegetation attains 70% uniform coverage the operator should remove the silt fencing.
2108284 Primary Facility 2012-10-17 Routine/Complete Inspection No Violations Noted Drilling had been completed, well appeared to be equipped for production. Brine tank located onsite and they appeared to be in sound condition. The site had been regraded, seeded and mulched, and it was noted that vegetation had begun to take hold. The operator should continue to monitor the site to insure that 70% uniform vegetative cover is achieved.
