Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1914196 Primary Facility 2010-09-17 Drilling/Alteration No Violations Noted
1914566 Primary Facility 2010-09-22 Routine/Complete Inspection No Violations Noted Drilling was ongoing at the time of the inspection. Below grade pit located onsite and it had more than the required two feet of lined freeboard. All erosion controls that had been implemented previously during site construction for the Ruth Iseman 3H (005-30493) had been maintained properly and were functioning to minimize accelerated erosion & sedimentation. Specifically the seeding and mulching of affected areas had significantly reduced the amount of disturbed area which could generate sediment. Access road had been heavily rocked for stabilization and no accelerated erosion or sedimentation was observed in relation to the access road. No erosion problems are anticipated during well development, which will require the site to remain open through the winter months, due to the erosion controls undertaken. Noted that the pipeline associated with the site had vegetation beginning to take hold on the reclaimed pipeline right of way.
1919385 Primary Facility 2010-10-13 Routine/Complete Inspection No Violations Noted Drilling was not ongoing at the time of the inspection. Below grade pit located onsite and it had more than the required two feet of lined freeboard. Above grade frac impoundments located onsite and it had more than two feet of lined freeboard. Observed no accelerated erosion and sedimentation in association with the access road or well-site.
1919958 Primary Facility 2010-09-08 Drilling/Alteration No Violations Noted drilling @ 350ft/E&S ok/ID&pit ok
1944002 Primary Facility 2011-01-19 Routine/Complete Inspection No Violations Noted Below grade pit located onsite and it had more than the required two feet of lined freeboard. Observed no accelerated erosion and sedimentation in association with the access road or well-site.
1947315 Primary Facility 2011-02-03 Routine/Complete Inspection No Violations Noted Drilling rig located onsite and drilling was ongoing at the time of the inspection. Observed no erosion & sedimentation issues with either the access road or site. Operator should continue to monitor all erosion controls to insure their continued functionality. Frac pits located onsite appeared to be predominantly frozen and had more than the required two feet of lined freeboard. Drill pit onsite as in use. Drill pit appeared to have a lot of frozen liquid. Rig was depositing cuttings and fluids into the pit. Noted that the pit was approaching the freeboard limit. Spoke with the company man while onsite and confirmed that should it be necessary there is a plan in place to utilize tanks to contain cuttings and fluids until weather permits removal of fluid from the drill pit.
1956066 Primary Facility 2011-02-17 Drilling/Alteration No Violations Noted
1958034 Primary Facility 2011-03-16 Routine/Complete Inspection No Violations Noted Drill rig had just completed moving from the Iseman well pad to the Lenape Heights pad. Observed no accelerated erosion & sedimentation in relation to the site or access road. Specifically there was no evidence of sediment transport off-site during moving the drill rig and ancillary equipment. Drill pit located onsite had more than the required two feet of lined freeboard.
2042766 Primary Facility 2012-02-15 Routine/Complete Inspection No Violations Noted Drilling had been completed, well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. Erosion controls had been implemented and maintained as outlined in the approved ESCGP. Well stimulation impoundments were observed onsite and they had more than the required two feet of lined freeboard.
2129270 Primary Facility 2013-01-10 Routine/Complete Inspection No Violations Noted Drilling had been completed, well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. Erosion controls had been implemented and maintained as outlined in the approved ESCGP. Well stimulation impoundments were observed onsite and they had more than the required two feet of lined freeboard.
2203323 Primary Facility 2013-09-06 Routine/Complete Inspection No Violations Noted Drilling had been completed, well appeared to be equipped for production. Brine tank located onsite, with containment and it appeared to be in sound condition. Erosion controls had been implemented and maintained as outlined in the approved ESCGP. Well stimulation impoundments were observed onsite and they had more than the required two feet of lined freeboard.
2252478 Primary Facility 2014-03-11 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the site or access road.
2325803 Primary Facility 2014-12-03 Routine/Complete Inspection No Violations Noted Drilling had been completed, well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the site or access road. Well stimulation impoundments were in the process of being emptied and the liner systems removed.
2391370 Primary Facility 2015-07-21 Routine/Complete Inspection No Violations Noted Well was appropriately labeled and brine tank located onsite. Brine tank appeared to be in sound condition. There was no observed accelerated erosion & sedimentation in relation to the site or access road.
