Drilling was to deepen an existing well to reach the Marcellus formation. Drill pit and frac impoundment had been constructed properly and were lined. Both the pit and the impoundment contained minimal fluids. The E&S plan rendering as compiled by Botsford Surveying had been significantly altered. The silt socks and the rock lined ditches had not been implemented. An additional soil stockpile had been implmented below the frac impoundment and created a sediment trap between the soil stockpile and the frac impoundment. The operator should monitor and maintain the installed erosion controls as neccesary to insure the minimization of accelerated erosion & sedimentation.

2828581 Primary Facility 2019-01-17 Routine/Complete Inspection No Violations Noted On 1/17/19 at 1210 hrs., I conducted a routine inspection of MDS Energy Development, LLC's Joe J Kuhns etux 1A-75 well (API 005-30485) in Wayne Twp., Armstrong Co. The well pad is restored and permanently stabilized. The well is equipped for production. A site ID sign is present at the entrance to the well pad. A well tag is affixed to the well head. There are no violations noted for this inspection.
