1828502 Primary Facility 2009-09-11 Routine/Complete Inspection No Violations Noted Site not constructed at the timem of the inspection. Due to the length of the access road, and the stream crossing which will require a Gp-5 and GP-7, the Department recommends that the operator rock the entire access road with a clean durable rock so as to minimize the amount of sediment generated by the traffic associated with well and well-site development. E&S controls for the permitted stream crossing should isolate the stream from the distured areas for well-site development. As per phone conversation with Tom Baun, the E&S plan drawing will be revised to show that the filter fence will be backed with haybales due to the proximity of the disturbed area to to the UNT of Spruce Run, and the drop off of elevation from the toe of the fill to the eastern stream bank.
1874518 Primary Facility 2010-04-02 Routine/Complete Inspection No Violations Noted Drilling was ongoing at the time of the inspection. Below grade pits located onsite and both of them had more than the required two feet of lined freeboard. Stream crossing had been implemented properly and no accelerated erosion & sedimentation was observed in relation to the access road. Noted that at the far back corner of the well-site the silt fence had been compromised, however no off-site transport of sediment had ocurred as of the time of the inspection.
2616238 Primary Facility 2017-07-18 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the site or access road. It was noted the well was located amidst a cultivated soybean field.
