1863478 Primary Facility 2010-02-17 Routine/Complete Inspection No Violations Noted Site constructed, drilling had not commenced at the time of the inspection. The rig crew was in the process of moving the rig and all ancillary equipment onsite. E&S plan located onsite and the two culverts which were two have been installed as part of the access road had been removed from the plan drawing and had not been installed. Additionally it was noted that the access road required additional clean durable rock to help stabilize the road and minimize sediment generation and transport during well development. Contacted operator representative Tom Baun and requested that there be clean durable rock placed on the entire access road due to the perpendicular intersection the access road makes with State Highway 1026. Mr. Baun stated that he will have the access road addressed as soon as possible.
1865029 Primary Facility 2010-02-23 Routine/Complete Inspection No Violations Noted Drill rig was in the process of being moved off-site at the time of the inspection. Noted that additional rock had been implemented on the access road as outlined in the 02/17/10 inspection. Noted a small amount of mud had been transported out onto the paved road, however a plow and broom truck was located onsite to address any mud that would make its way onto the paved road. The operator should evaluate all site erosion controls as the snow melts for functionality and make any necessary repairs. In addition the operator should address the access road as necessary prior to any well stimulation activities.
1867482 Primary Facility 2010-03-04 Routine/Complete Inspection No Violations Noted Drill rig had been moved off-site at the time of the inspection. A below and above grade pit were located onsite. The above grade pit did not contain any fluids at the time of the inspection. The below grade pit had more than the required tow feet of lined freeboard. However there were some areas where the pit liner had begun to fall down in. The drill cuttings should be disposed and this pit closed as soon as possible. Observed no accelerated erosion & sedimentation on the well-site, however there were several areas where the silt fence had been knocked over due. The silt fence should be addressed as soon as possible.
1965645 Primary Facility 2011-04-14 Site Restoration No Violations Noted Drilling was completed on 02/23/10. Well appeared to be equipped for production. Brine tank located onsite and appeared to be in sound condition. Site had been properly restored and no accelerated erosion or sedimentation was observed in relation to the site and access road.
2213404 Primary Facility 2013-10-17 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the site or access road.
2650152 Primary Facility 2017-10-20 Routine/Complete Inspection No Violations Noted This well is tagged and in production. The well head casing and grounded 100 bbl plastic tank look ok.
