The E&S plan as compiled should effectively minimize accelerated erosion & sedimentation provided the plan is implemented & maintained properly (i.e. culvert BMP's are installed as outlined in the details included in the submitted E&S plan). In addition the operator should monitor the base of the fill slope, piped portion of the stream, and filter fence backed with hay bales to insure they are all functioning properly, and the fill slope is remaining stable.
1895732 Primary Facility 2010-07-02 Routine/Complete Inspection No Violations Noted Site constructed, drilling had not commenced. Access road and associated erosion control BMP's were still under construction. E&S plan for the well-site had been implemented as approved under the OG-57 waiver obtained. The Department recommends that the operator endeavour to complete drilling, well stimulation, and/or any reclamation activities to minimize the amount of time the site is open and exposed thereby minimizing the potential for issues to develop with the site and adversely affect the UNT to the South Branch of Pine Creek.

1913923 Primary Facility 2010-09-10 Routine/Complete Inspection No Violations Noted Drilling had been completed, well appeared to be equipped for & in production. brine tank onsite and appeared to be in sound condition. The site had been regraded, seeded & mulched. Noted that the site did require additional mulch to be applied. Noted that the culvert that had been in place to convey the stream during well development had been removed and the area directly adjacent to the stream had been stabilized with clean durable rock. No accelerated erosion or sedimentation was observed in relation to the site or access road. The operator should monitor the site to insure that the vegetative cover takes hold before winter or insure through temporary BMP's that the site remains stabilized until 2011's growing season.
1952344 Primary Facility 2011-02-25 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tank onsite and in sound condition. Observed no erosion issues in relation to the access road or well-site. Site had been regraded and vegetation was observed onsite. Operator should evaluate the site to insure vegetation is continuing to grow and the access road to insure that the permanent erosion controls are continuing to function properly.
2099460 Primary Facility 2012-09-12 Complaint Inspection No Violations Noted Well appeared to be equipped for production at the time of the inspection. Brine tank located onsite and it appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the access road or well-site.
2109871 Primary Facility 2012-10-18 Routine/Complete Inspection No Violations Noted Drilling had been completed well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. Site had been restored and the vegetative cover met the 70% uniform coverage criteria.
