2005367 Primary Facility 2011-09-23 Routine/Complete Inspection No Violations Noted Well was spudded on 10/14/09. Drilling had been completed, well appeared to be equipped for production with a pumpjack apparatus and a brine tank. Brine tank appeared to be in sound condition. Drill pit located onsite and it contained drill cuttings, frac sand, and fluids. There were several small holes in the pit liner observed, and the fluid levels were below the holes. The holes were located in the left corner of the long side of the pit furthest away from the well apparatus. There was no accelerated erosion or sedimentation observed in relation to the access road or well-site. The Department recommends that the operator take action to remove the fluids from the pit and begin pit closure activities as soon as possible. At this time the Department is requesting that the operator forward documentation stating how long the pit has been located onsite and what the timeframe is for closure of this pit. This documentation should be sent to the attention of WQS Gregory Higgins no later than October 18, 2011.

Brine tank located onsite and it appeared to be in sound condition. Drill pit and well-site had been regraded and the site had been mulched. The operator should continue to monitor the site to insure that it attains 70% uniform vegetative cover and for accelerated erosion and/or sedimentation until 70% uniform cover is achieved.
