1765463 Primary Facility 2008-11-19 Routine/Complete Inspection No Violations Noted Found the following deficiencies with the E&S plan drawing: 1) Due to the slope of the area below the site, and the proximity of the site to SR 2005 and the unamed tributary to Cowashannock Creek, the operator should utilize filter fabric fence reinforced with hay bales. 2) The operator should show on the E&S plan drawing what BMP's are to be utilized to minimize accelerated erosion & sedimentation at the culverts inlet & outlet, as this culvert will be hadnling all the runoff from the 300+/- feet of new access road. In addition the Department reccomends that the operator rock the entire access road so as to minimize the amount of sedimentat generated and transported on the access road.
1859111 Primary Facility 2010-01-26 Routine/Complete Inspection No Violations Noted Site is located less than 100 feet from an UNT of Cowanshannock Creek. A waiver of the distance requirement had b een applied for and approved on 01/29/09. The site had been constructed and erosion controls had been implemented as outlined in the approved E&S plan. Drilling had not commenced mat the time of the inspection.. Operator was in the process of moving the drill rig onsite. Due to the current weather conditions WQS Higgins contacted operator representative Dan Rice to insure that there would be a plow and broom truck available. Mr. Rice stated that plow truck will be available within 2 hours should it be required onsite. It was observed that there were a few rocks located within the below grade drill pit. WQS spoke with rig crew onsite who stated that the pit will be lined in accordance with 78.56.
1874507 Primary Facility 2010-04-02 Routine/Complete Inspection No Violations Noted Drilling had been completed, well appeared to be equipped for production. Brine tank located onsite and appears to be in sound condition. Site had not been regraded, seeded, or mulched. Observed no accelerated erosion & sedimentation with respect to the access road or well-site. The operator should complete final restoration activities as soon as possible.
1981711 Primary Facility 2011-06-20 Site Restoration No Violations Noted Drilling was completed on 02/02/10. Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. Observed no accelerated erosion or sedimentation in relation to the access road or well-site.
