1750107 | Primary Facility | 2008-10-22 | Routine/Complete Inspection | No Violations Noted | Found the following deficiencies with the submitted E&S plan: 1) The rendering of the access road does not show the upgrades & BMP's that will be neccesary to utilize the existing stream crossing and the existing access road. In addition the stream crossing would most likely require a GP-11 permit as that permit covers upgrades to exisiting crossings. 2) To minimize sediment transportation on the access road, the road should be rocked approaching the stream crossing from either side. 3) The plan should utilize other BMP's as neccsary to isolate the access road and crossing from the stream. 4) The filter fabric fence should be reinforced with hay bales due to the proximity of the site to the stream. 5) Noted an exisiting well less than 1,000 ft from the proposed well. The well-site appeared to have been freshly regraded and the production equipmment appeared to be new equipment. A closer inspection of the existing well was not performed at that time. |
