Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1735772 Primary Facility 2008-09-04 Routine/Complete Inspection No Violations Noted Well staked, site not constructed. E&S plan drawing revised to show BMP's at culvert outlets and filter fence backed with hay bales. E&S plan should minimize accelerated E&S if plan is impemented & maintained properly.
1820754 Primary Facility 2009-08-14 Routine/Complete Inspection No Violations Noted Drilling was completed, well appeared to be equipped for & in production at the time of the inspection. Brine tank located onsite and appeared to be sound condition. Spoke via phone with operator representative Dan Rice regarding a diversion ditch located above the site. The ditch required further stabilizion to function properly carrying runoff and minimizing acelerated erosion & sedimentation. Mr. Rice stated that the first of the week beginning with 08/17/09 he will have erosion control matting placed with seed and mulch to stabilize the ditch and monitor the site to insure that the erosion control measures are functioning properly.
1821492 Primary Facility 2009-08-19 Routine/Complete Inspection No Violations Noted The issue with the diversion ditch observed during the 08/14/09 had been addressed and corrected by the operator. Erosion control matting had been placed and the areas devoid of vegetation had been seeded & mulched. In addition hay bales had been implemented on the downhill slope of the ditch and at the bottom of the fill slope for the original well-site as a temporary BMP until the vegetation began to grow and take hold. The operator should continue to monitor the site to insure that the additional erosion controls function properly, and the vegetation begins to grow.
2252830 Primary Facility 2014-03-12 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production at the time of the inspection.
